🚨 Managing Status Epilepticus: From Initial Treatment to Advanced Care 🚨 Status Epilepticus (SE) is a medical emergency requiring rapid intervention. Here’s a step-by-step guide to its management: 1️⃣ Initial Care (0-5 minutes): Ensure airway, breathing, and circulation (ABCs). Administer oxygen and establish IV access. Correct hypoglycemia if needed. 2️⃣ First-Line Treatment: Benzodiazepines: Lorazepam: 0.1 mg/kg IV (max 4 mg), repeat after 10-15 mins. Diazepam: 0.15 mg/kg IV or 0.2 mg/kg rectal. Midazolam: 0.1-0.3 mg/kg IV or 0.2 mg/kg intranasal. 3️⃣ Second-Line AEDs: Phenytoin: 20 mg/kg IV. Valproate: 20-40 mg/kg IV. Levetiracetam: 20-60 mg/kg IV. 4️⃣ Refractory SE: Continuous infusions of Midazolam, Propofol, or Pentobarbital. If these measures fail and the condition escalates to Super-Refractory Status Epilepticus (SRSE), deeper sedation and alternative treatments like immunotherapy, ketogenic diet, or even surgical interventions may be necessary.
