While the Patient Safety Act makes provisions for #mandatory #opendisclosure of #13notifiable incidents, the majority of which involve the death of a patient, it is important to remember that open disclosure is required following all patient safety incidents where patients have experienced any level of harm, including minor harm. Patients have a right to be fully informed and to receive an explanation and a sincere and meaningful #apology. The open disclosure process will ensure that learning occurs which will ultimately lead to service improvement and improved #patientsafety.

As my colleague Rachele Chaar, MSc and I were discussing her recent bloodborne pathogen education event, the topic of sharps’ risk sparked our preventionist mindset. Sure enough, the below infographic by APIC popped on my LI feed today 👍 and is a great resource to be shared. I will add another resource, this with a Canadian focus, by the Canadian Centre for Occupational Health and Safety. Link below in comments. Diving into the CCOHS resource, let’s talk about risk source. These sharps injuries (creating a cut in the skin, which allows contact between blood or fluids) gives the potential to transmit infectious diseases, especially blood-borne viruses. Concern includes the human immuno deficiency virus, hepatitis B, and hepatitis C. 🦠Note: The risk of infection after exposure to infected blood varies by blood-borne pathogen. Some interesting stats on where sharp risk occurs!👀…. ➡️ the Ontario Hospital Association in 2016 estimate that after an injury in workplace situations from a needle contaminated with hepatitis B virus, there is a 6 to 30% chance that an exposed person will be infected. In a similar situation with HIV, there is about a 0.3% chance of infection, and there is about a 1.8% chance of infection for hepatitis C. ➡️ injuries from sharps can occur at every stage of their use, disassembly, or disposal. ➡️ When thinking of needles, the greatest risk is during use at 52%, with disposal, coming in second at 22%. 🧐 e.g., in transit to disposal, improper disposal, or even during disposal itself. ➡️ for solid sharps, during use, at 70% is the greatest risk. Check out the site for full list on risk examples and percentage breakdown. 💡 How can needlestick and sharps injuries be prevented? #Prevention is the most effective way to protect workers. A comprehensive sharps injury prevention program would include: 🔵 recommended guidelines, 🔵 improved equipment design, 🔵 effective disposal systems, 🔵 employee training, 🔵 safe, recapping procedures, when necessary, 🔵 surveillance programs. Preventing injuries from sharps and needle stick is considered a part of the #routinepractices used by all healthcare workers. 👏 #healthcare #occupationalhealth #workersafety #infectionprevention #educate
