This sounds fantastic. As a PCP - and recently as a patient - I have experienced the incredibly frustrating fragmentation of our health care system. I’m excited to see you’re targeting this. Patients and physicians will be better off for it.

Congrats Rahul — love the mission. This is very exciting.
Great step towards providing care to low income patients. I am glad you guys focusing most underserved and vulnerable populations. But for continued care cost is a factor, more challenging is patient engagement, care efficiency and care satisfaction.
It's wonderful to see a high-acuity, high unmet need, and low visibility population get this type of holistic and wrap-around care. Congrats and rooting from afar!
