It's remarkable that one of the most fundamental design tools for empathy has evolved so far in two decades of practice. Journey maps are still anchored on human experience, but the most sophisticated ones now reveal real opportunities for system leverage, including policy change. Every time an individual encounters a challenge in self-navigating their care experience is an indication of where system gaps exist. I had the privilege, on the panel, of referencing this work on congenital heart disease, led substantially by the team I led at the Design Institute for Health (http://bit.ly/3PG2t2J). Where in your work are you leveraging a journey map to challenge the present system?
The Alliance for Health Policy's first panel of the day delved into #patientjourneymapping and how journey mapping can be used to improve #patientoutcomes and #experiences. Thank you to our fantastic speakers who brought this panel in our "2023 Signature Series: Envisioning a Person-First Health System Summit" to life: Moderated by: Hala Durrah, MTA, Patient Family Engagement Consultant, Independent Consultant Stacey Chang, M.S., Principal, New Origin Studio Jen Horonjeff - “Ask Patients”, Ph.D., M.S., Founder and CEO, Savvy Cooperative Eliot Fishman, Director of Policy and Programs Group, Center for Medicare and Medicaid Innovation, Centers for Medicare and Medicaid Services #healthcare #healthpolicy #innovation

It's Global #HealthEquity Week, and we're here for it! Much of our work with State Medicaid Agencies and Health Information Exchange lifts up the principles of social determinants of health and health equity in pursuit of improving the access and delivery of care. [Quote reads: "Recognizing the opportunity to leverage innovation and partnering with local communities and the private sector to support Medicaid programs excites me about Medicaid in 2024! Especially in the SDOH space. I think we will see a lot of transformation in this arena."]

Individuals dually eligible for Medicare and Medicaid are more racially and ethnically diverse than the general population of Medicare-only beneficiaries. This means that racial and ethnic health care disparities disproportionately affect dually eligible individuals. In Resources For Integrated Care's recent podcast episode, Dr. Romilla Batra ,former Chief Medical Officer of the Medicare Advantage health plan SCAN, shares insights about the not-for-profit's initiatives to reduce health care disparities and data-driven strategies to advance health equity. The Physicians Foundation recently launched a campaign called "Let’s Take 5 to Address Drivers of Health," marking a significant shift in health care. For the first time, physicians can report two non-medical drivers of health measures to the U.S. Centers for Medicare & Medicaid Services. This is a game-changer. At EHF, we've always advocated for a health care system that goes beyond treating symptoms to also addressing the underlying root causes of poor health. By focusing on prevention and tackling non-medical drivers of health like access to quality health services, nutritious food, and safe environments, we can make all communities healthier. Join us as we work to shift the health system toward a more equitable and preventive model that's focused on improving #HealthNotJustHealthCare. #NMDOH #SDOH #philanthopy https://lnkd.in/dYk-2dCd

How can we build a stronger, more sustainable health care system that ensures greater access for all Americans? Yesterday, I joined a panel of experts from across the health care industry to tackle that question and more at the AHIP Medicare, Medicaid, Duals & Commercial Markets Forum in Baltimore. Thanks to Krista Hoglund, John Kao, Talya Schwartz, and moderator Michael Tuffin for the insightful discussion about the many challenges facing our industry, and how we can move forward together. #Health #HealthCare #HealthCareOnLinkedIn
