Blimey it's really sad and sobering to see the types of errors here and that they're not limited to distant reports that happen elsewhere but regularly occur not only in my own area, but I'm sure every area. The lessons and warnings haven't changed much over the last decade of my time as qualified social worker yet why do social workers, GPs, psychiatrists, district nurses etc and all the employing organisations and leaders keep making the same mistakes with impunity? I've had terrible experiences and been bold enough to suggest intervention and working together is better than having to learn lessons often to gormless stares in the face if exact themes highlighted here.
It's important to reflect honestly otherwise there is no point of any cpd and though it shouldn't, it feels brave to put head above the parapet and do that honestly in a public forum. If we can't however what's the point? I also commend the openness and transparency of Bexley and agencies invovled for the depth and honesty shown. One of best pieces of cpd I've read for a long time.
