Even for us with dementia, I only at best know how my buddy 'in crime' Anjang who is living with Parkinson and LBD from his regular sharing with me when we catch up. Knowing about the symptoms only means that you are aware of it. It is living with the condition 24/7 that make an individual the Expert of the subject matter beyond clinical knowledge. Always have Experts by Experience to be speaker(s) of your conference/event to have a holistic understanding of dementia or disease specific. In the healthcare journey, often the hardest thing is coming up with a plan, particularly when the patient receives a life-altering diagnosis. The importance of considering end of life is being highlighted across acute, community, primary, and aged care. The 2nd edition of the National consensus statement notes its scope as: The Consensus Statement applies to all services where health care is provided to people approaching the end of their life, including hospitals, hospices, residential aged care facilities and home settings. The recently released final draft of the Aged Care standards includes clinical care with Standard 5 describing the responsibilities of providers to deliver safe and quality clinical care to older people including palliative care and end-of-life care. Both residential aged care and homecare are providing care to older people in the last days, months and years of life. It is worth considering how you can address the processes and issues outlined in these documents. For services and staff needing knowledge and skills the Australian Government of Health and Aged Care has invested in national programs to support the sector including palliAGED, ELDAC, CareSearch, and CarerHelp. There are free resources for the workforce, for services, for older people, for carers and for family and friends. Beyond the documents, we have the opportunity and responsibility to provide older people with compassionate and skilled care as they approach the end of their life and to support their family and those providing care.
