Two recent unrelated studies published in Health Affairs Scholar revealed that Z-codes are being vastly underutilized. Understanding and addressing social determinants of health is imperative to achieving health equity. We researched 10 of our customers with an expansive dataset of 1.4 million unique patients from all claims data from 2022, finding that merely 1.28% of the total patient cohort had documented z-codes. A comprehensive strategy is needed to address the impact of social determinants of health. Racial and ethnic inequities in health care identified 20 years ago in the Unequal Treatment: Confronting Racial and Ethnic Disparities in Health Care report have remained largely unchanged. Urban Institute’s Unequal Treatment at 20 Initiative is addressing this stagnation by identifying promising solutions that decisionmakers, practitioners, and advocates can use to create a more equitable US health system. Efforts to address persistent inequities are influenced by and require consideration of the sociopolitical context within and outside health care. In this report from Urban, our experts explore the past and present-day sociopolitical climate and our current progress on creating a more equitable US health system.
