🎥 “Real-time Recovery from Anaesthesia – A Game-Changer!”🧠💉 In a recent case, we successfully managed a patient with acute-on-chronic subdural hematoma (SDH) who presented with severe headaches and was unable to even sit up due to pain. Using advanced intraoperative techniques, including “Total Intravenous Anaesthesia (TIVA)”with ‘Propofol’ and ‘Remifentanil’, we ensured the safest and most effective anaesthesia. 🔬 Key components of anaesthesia monitored: 1. EEG - Depth of Anaesthesia 2. NMT - Neuromuscular Monitoring 3. SPI - Intraoperative Analgesia Monitoring Following a minimally invasive ‘endoscopic evacuation of the SDH’ I had the privilege of witnessing a remarkable outcome. The patient was not only fully awake but was able to “walk out of the operating theatre” right after surgery! This is the power of ultra-short-acting agents like Remifentanil, combined with the precision of Sugammadex to reverse muscle relaxation instantly. 📈 Patients and their families often ask, "When will the patient wake up from anaesthesia?" Excited to share our latest research published in the Journal of #European Archives of Oto-Rhino-Laryngology (Publisher: #springer). Our study evaluated cerebral venous sinus thrombosis (CVST), a potential complication post-surgical treatment for vestibular schwannoma. In our #metaanalytic approach, we sought to assess the prevalence of CVST in patients undergoing surgical treatment for vestibular schwannoma. Our study revealed a pooled prevalence of CVST after vestibular schwannoma surgery at 6.4% (95%CI 3.4–11.5%). Further exploration of different surgical approaches unraveled intriguing findings. The pooled prevalence of CVST following retro sigmoid (RS), translabyrinthine (TL), and middle cranial fossa (MCF) approaches were identified as 4.8% (95%CI 2.0–11.0%), 9.6% (95%CI 4.3–20.3%), and 9.9% (95%CI 1.6–42.2%), respectively. Notably, a significant difference emerged between the TL and RS approaches, with an Odds ratio of 2.10 (95%CI 1.45–3.04, P < 0.001). The #sigmoid sinus exhibited the highest #post-operative #thrombosis rate at 7.9%, surpassing the transverse sinus (3.7%) and the involvement of both sigmoid and transverse sinuses (1.6%). I am pleased to announce the publication of our latest article titled "Hand Digit Revascularization: Could Be an "Elective-Urgence" Surgery?" in Journal of Clinical Medicine 🚀 🔬 Abstract: Timely access to digital replantation surgery has traditionally been considered crucial for success. However, our case study challenges this paradigm. We successfully performed revascularization after 72 hours of warm ischemia, demonstrating that delayed interventions can still result in a good recovery of motor and sensory functions. This case highlights the need to reevaluate current ischemia time limits and could open new avenues for treatment protocols, expanding access and improving the quality of care in digital replantation. Key Points: Successful revascularization after a prolonged warm ischemia period. Significant recovery of hand function and clinical outcomes over one year.
