In 2022, I presented at CHOP Cardiac Grand Rounds. I bared my soul to my 900+ cardiac colleagues, sharing my experiences as a child undergoing multiple open-heart surgeries, hospitalizations, painful procedures, etc., and how all that trauma led me down a dark path and an eventual diagnosis of complex PTSD. Why did I share my secrets of some of the worst times throughout my life? Simply because I want better for all pediatric cardiac patients that come after me. I wanted clinicians to *truly* understand what we go through; how it affects our relationship with our own bodies, the lack of bodily autonomy, and the mental health implications that haunt so many of us throughout our lives. That often times, the gift of being “saved,” is also a curse. My own transition to adult congenital cardiac care was my peds cardiologist telling me at age 25 (26? I’m old now, I can’t remember), “next year you’re going to see the ACHD team at Penn.” That was it. And the following year, I entered “The Dark Side” and left to navigate on my own. There had to be a better way. It took a bit longer than I hoped (that’s healthcare for you), but I am officially the CHOP Transition Readiness and Awareness for Cardiac Kids (TRACK) Nurse Coordinator. This is my professional nursing dream come true- I’ve been talking about it for close to two decades. Early Mobility: Part 2 “Why isn’t he extubated?” This is the most common question asked when these videos are shared. It is an understandable question from the perspective of an ICU community that only expects patients to be awake once it’s time to assess for extubation criteria. Yet this exposes how backwards our culture and protocols are. If we approach mobility as a “back-end”, “rehabilitation”, “clean-up” intervention then we will constantly be battling ICU-acquired weakness and delirium. Mobility is a vital tool to PREVENT these complications IF used early on. Timing of mobility is key to make the biggest impact on patient outcomes and sustaining an early mobility program. It is too labor intensive and depressing to only be waking patients up and trying to move them once they are severely weak, confused, and tortured. This patient is not extubated because he needs mechanical ventilation. He is awake and walking because his team understands that having sick lungs does not mean he must leave with a new brain injury and neuromuscular condition. They know that he must be mobile within 72 hours after intubation to have any impact on preventing post-ICU syndrome. They know that walking within 48 hrs after intubation will improve his cognitive function by 20% 1 year after discharge. They know that this will protect him from ventilator-associated pneumonia by over 40%. They see the ventilator the same as a walker. It provides more support to make mobility possible. They know that this is how to save lives in the ICU. I’m proud to share another success story from our team demonstrating the life-saving potential of the BEFAST assessment protocol. Recently, one of our vigilant nurses identified stroke symptoms in a patient attendee experiencing giddiness. Thanks to her quick assessment and the BEFAST protocol, the attendee was immediately transferred to the ER, diagnosed with Acute Ischemic Stroke, and received timely thrombolysis. This is the second such incident where early stroke identification by our nurses led to prompt and effective treatment. Why BEFAST? The BEFAST protocol is designed to help healthcare professionals quickly identify the early signs of a stroke, ensuring timely and effective intervention. Here’s a quick overview: • Balance: Sudden loss of balance or coordination • Eyes: Sudden vision changes • Face: Facial drooping • Arms: Weakness in one arm • Speech: Difficulty speaking • Time: Act quickly and call emergency services By incorporating the BEFAST assessment into our nurse's daily routine Assessment (Every Shift assessment), we are taking proactive steps to: 1. Enhance Early Detection: Identify stroke symptoms at the earliest possible stage. 2. Improve Patient Outcomes: Ensure patients receive the fastest possible treatment. 3. Empower Our Nurses: Equip our nursing staff with the tools and training needed to save lives. It’s high time we emphasize the importance of the BEFAST assessment protocol, not just within healthcare settings but across our entire community.
