Congratulations to our newest The DAISY Foundation award winner Yum Truong, from our Med/Surg team at #AdventistHealthUkiahValley. Her peers recognized her advocacy for a patient, making sure they had all the resources needed for continued recovery. Her nomination reads, “A patient was discharged with 5 new medications as a Spanish speaking patient who did not understand her discharge instructions. Knowing that it was very important for the patient to be able to understand her home care instructions, Yum jumped into action gathering resources, even going as far as printing pictures of pills, showing which ones to take at which time, partnering with the pharmacist to help educate the patient. She deployed Meds to Beds, calling anyone to help translate in person and working with the patient's friend who spoked the language to help translate at the bedside. Yum stopped at nothing to ensure the patient had everything they needed before discharge. It would have been easy to go and discharge the patient with a video interpreter. But Yum took the extra step to provide the patient with resources so that they can be successful in their care after being discharged.” Congratulations Yum!
