Dr. Raj Singh M.D., the primary care physician at HEALOR, a Las Vegas-based holistic primary care clinic, and the medical director of RAREV medspa, was recently featured on KTNV Channel 13 Las Vegas. In the news segment, one of Singh’s patients was profiled for his successful weight-loss journey using the medication Mounjaro. Of greater focus in the story, however, was Singh’s views on the dangers of weight-loss drugs being available online without a prescription. “I think the biggest [concern] is how easily they are available without proper bloodwork or a proper exam,” Dr. Singh said, while adding that getting your heart, kidney and liver function checked before taking the medications is critical. “[For example], there’s a huge risk of developing gallstones if you are losing weight too rapidly. So, we have medications we use prophylactically so you don’t develop gallstones.” Blood clots are another potential side effect of the medications, Singh added, further emphasizing the need for medical oversight when using the drugs. He also said receiving fake pills or improper dosages from online sources is another risk. Even with these dangers, demand for the drugs continues to explode. Based on the current limited evidence, MTM (Medication Therapy Management) programs in older adults appear to have a positive impact on reducing PIM (potentially inappropriate medication) use. However, evidence was limited by study design, the lack of consistency in outcome measures, and a short follow-up period. Future work should adjust for confounding, apply comparator groups, include longer-term outcomes, and develop a core set of measures that can be consistently applied across studies. https://lnkd.in/eu8D3_vj

In March 2020, Calvin Meaney, PharmD, interim division head, Division of Outcomes and Practice Advancement and clinical associate professor, #UBuffalo Department of Pharmacy Practice, met with a group of nephrology pharmacists to discuss ways to help improve patient lives and keep the nephrologist at the center of the patient care team. From those discussions, the Advancing Kidney Health through Optimal Medication Management (AKHOMM) initiative was formed and Meaney now serves as an assistant director. The vision of AKHOMM is every person with kidney disease receives optimal medication management through team-based care, including a pharmacist, to ensure their medications are safe, effective and convenient. Recently, Meaney and the AKHOMM team concluded that pharmacist-led optimal medication management is achievable within a value-based care model. This approach involves pharmacists taking a central role in managing patient medications, which improves medication adherence and patient health outcomes while also reducing overall healthcare costs. They found integrating pharmacists into nephrology and primary care practices can help empower patients to be partners in their medication management which can help decrease nephrologist workloads.
