For both paediatric and adult patients, high-flow or high-velocity respiratory support is proven to extend the time before desaturation during intubation. Recognizing the critical importance of correct and timely diagnoses in ensuring patient safety, “Improving diagnosis for patient safety” has been selected as the theme for World Patient Safety Day 2024. The global action plan encourages countries to adopt strategies that reduce diagnostic errors, which often arise from a combination of cognitive and system factors that impact the recognition of patients’ key signs and symptoms, and the interpretation and communication of their test results. (World Health Organization WHO) “When the diagnosis is correct, the prescribed medications will be appropriately targeted, and this will be directly reflected in the patient’s clinical manifestations, which we observe due to our proximity to the patient, including pain thresholds, temperature, and lab results. For example, diabetic coma requires differential diagnosis among various diseases that present with similar symptoms, such as shock and strokes. A correct diagnosis allows us to use only intravenous glucose solutions, resulting in noticeable and rapid improvement in the patient’s condition. Diagnostic errors, harm, and death in US hospitals 10-second takeaway Diagnostic errors are a major contributor to adverse outcomes, but the prevalence and cause of these errors are not well defined. In this study, 23% of hospitalized patients who were transferred to the ICU and/or died experienced a diagnostic error; the most significant errors related to patient assessment and ordering and interpreting diagnostic tests. Targeting these key areas is crucial for quality improvement initiatives aimed at reducing patient harm.

📢 New Patient Safety Supplement (PSS) on medical device related pressure ulcer available now. It has been developed in collaboration with HSE National Improvement Programme in Wound Management and a patient with lived experience. A pressure ulcer (PU) can have a serious negative impact on a person’s quality of life, affect their mental health, cause pain and can lead to infection. Resource implications can be significant including increased length of hospital stay. This Patient Safety Supplement (PSS) focuses on medical device related pressure ulcers (MDRPU). It aims to support healthcare staff and patients/service users to identify the risk factors for developing MDRPU and reiterate how everyone can be proactive in helping to prevent these often avoidable injuries.

As a pharmacist who has worked with critically ill patients, the current shortage of intravenous (IV) fluids hits close to home. This situation challenges our ability to provide optimal care and raises concerns about patient safety. We often rely on precise fluid management to maintain stability in our patients, and the unavailability of essential IV fluids can lead to treatment delays and increased risks. Effectively managing this shortage necessitates a heightened level of collaboration within our healthcare teams. This involves adjusting treatment protocols and maintaining vigilance in monitoring alternative therapies. It's a stark reminder of the resilience and adaptability required in our profession as healthcare professionals...
