Located anywhere, these AVMs(Arterio-venous malformations) bring a sense of serpentine high flow blood channels, pulsating and creating a sense of scare not only for the patient, but for the clinician also. The brain surgeons find themselves solitary at the ridge, where one side is the normal brain parenchyma and the other side is the whirling blood channels. Nevertheless, we tread cautiously in these cases for surgical excision, proving our mettle, as the present era has rising trend of Gamma Knife Radiosurgery and dying art of embolization. My 100th case of AVM was a fortish gentleman with SM Grade 4(size 6 cm, both deep-superficial venous drainage, right frontal lobe). Successful surgical excision providing permanent cure keeps surgery as the gold standard management option even today. MONDAY OF ENDOSCOPIC ENDONASAL SKULL BASE SURGERY 📍 Patient: 55-year-old man 🩺 Symptoms: Headache, nasal obstruction, and nasal voice. 🧠 Brain MRI shows an aggressive tumor with extension to the base of the skull, presenting clinical and radiological characteristics of high aggressiveness. ✅ Thanks to extended endoscopic endonasal surgery, we are able to address this type of complex lesions in a minimally invasive manner, offering immediate post-surgical recovery. What do a carotid endarterectomy (CEA) and crani for Chiari Malformation have in common? You can use neuromonitoring to help determine the next step in your surgery. Be it selective shunting or selective duraplasty, there's evidence for its utility as part of safer patient management. For CEA, you look for no change in EEG and SSEP = adequate blood flow and no shunt needed. For crani for Chiari Malformation, a reduction in conduction times and ABR and SSEP = adequate release after bony decompression and opting to forgo duraplasty as an option. #neuromonitoring #neurosurgery #surgery #vascularsurgery
