*Singapore’s Healthcare Evolution: A Shift Towards Value-Based Care* Last week, I looked at Singapore’s transformative healthcare initiative, “#HealthierSG”. This reform is a testament to Singapore’s commitment to transitioning towards value-based healthcare. It’s built on five foundational pillars: 💚*Preventive Care*: Mobilizing the primary care network with a strong emphasis on #prevention. 1️⃣ *Personalized Health Plans*: Development and annual monitoring of individual health plans. 🎪*Community Engagement*: Activation of community partners and implementation of “#social #prescribing”. 📋 *National Enrollment Program: A comprehensive enrollment program offering incentives for participants. 📶 *Infrastructure Development* Establishing the necessary infrastructure for HR, Funding, Management, and IT. What stands out in this reform is the strategic use of Key Performance Indicators (KPIs). Short Term: Process KPIs take precedence. Medium term: Focus shifts to financial and outcome-based KPIs. Longer term: The plan is to narrow down to a few critical #outcome-based KPIs such as clinical #frailty scores, prevalence of #chronic diseases, and incidence rates for cancer, stroke, and acute myocardial infarction. This progressive approach to healthcare reform in Singapore is a fascinating case study in the shift towards value-based care. Ethiopia has close to 126527060 people and these are served by 25743 health facilities. Unless there is a resource sharing mechanism between the rural and town based service providers ,equitable access of healthcare services can be hard to achieve in such a setting . Months back, through a partnership we opened our hub and spoke telemedicine to 90 rural health facilities (Pilot ) and we hope to get convincing results to allow us to scale to the whole country .Source ;Medic East Africa
