In addition, antigens are also present on many patient's red cells...i.e. kidd, kell, Duffy, etc...and must be considered prior to transfusion of packed red cells. This is often referred to as a screen...hence the order term, Type and Screen.
ABO is the king of all blood groups and leader of the Blood Group Kingdom. ABO is the oldest of all blood groups that was discovered and reigns over transfusion reactions. It's the first blood group to develop and is powerful and destructive because it's everywhere. ABO/Rh typing of specimens includes ABO grouping and Rh typing. ABO grouping of red cells, using commercial anti-A and anti-B antisera to determine the presence or absence of the antigens, is referred to as front or forward grouping tests. Plasma/serum grouping tests, using commercial A1 and B red cells to detect serum anti-A and anti-B, are referred to as reverse grouping tests. Routine ABO grouping of patients 5 months of age or older must include both forward and reverse typing. Routine Rh typing refers to the typing of red cells with anti-D antiserum to determine the presence or absence of the D antigen. Rh typing is performed on all patient samples as part of the request for ABO/Rh typing (blood type). I prefer the tube method because this is the gold standard. The tube method may be used to resolve any typing discrepancies discovered on the instrument. Can you see the agglutination on both A and B? This patient is AB Negative. #bloodbank #AMT #ASCP #ABO

Blood Bank tips: If you have a request for PRBCs and the patient shows an autoimmune condition, screening red cells antibodies negative and DAT is positive. by Gel card method: No compatible blood unit was found! The answer is to do the tube method with the following procedure: 1- Two drops of patient clotted serum in plain tube. “ red top tube serum is preferred “ 2- One drop of donor cells ( 3-5% suspension). 3- Incubation in a water bath for 15-60 min. “ in case you will use bovin no incubation needed due to bovine inhance capability” 4- Wash cells with Saline for three times. 5- Add two drops of AHG then mix gently. 6- Centrifuge for 20 seconds on 750 rcf. 7- Observing any agglutination under microscope. 8- Add your O check cell as a positive control to see if your negative result is valid. The procedure source is AHG GRIFOLS insert sheet. Bovine is an inhancer for reactions in tube method. Hopefully this will help you out in case you have an autoimmune patient. Cryoprecipitate is likely to be at least as effective if not superior to fibrinogen concentrate or cryopreserved platelets, in severe thrombocytopenia.
