An inspector found 37% prescription error rate in a community pharmacy 🚩 Wow. Disturbing revelations from a CVS Pharmacy resignation letter shed light on concerning working conditions and patient safety issues. A state investigation in Virginia Beach uncovered a staggering 37% error rate in prescriptions, with some errors posing significant risks to patients. Employees described intense pressure from corporate mandates, leading to what they described as 'sweatshop conditions.' It's no surprise that the third largest cause of all premature deaths is from medical errors. Ultimately, the biggest cost is on the patients. These findings underscore the critical importance of prioritizing patient safety and ensuring adequate support for pharmacy staff. Pharmacists are undeniably skilled at leveraging their expertise to ensure medication safety, but they're also human and susceptible to oversight. On the other hand, technology excels at identifying errors through systematic analysis and algorithms, however, it may miss nuances that a human pharmacist could catch. But when pharmacists and technology collaborate, error detection can reach near perfect accuracy. We may disagree on this, but I'm a firm believer of: "Inherit the patient. Inherit the medication list." It may seem a bit much but I'm putting it out there, with love, for all outpatient prescriber specialties, whether virtual or in-person. Anytime a change (+/-) is made to a patient's medication list, there's a chance that the patient's safety (with medication use) is unintentionally compromised if not properly assessed due to: -increased side effects -drug-drug interactions -altered therapeutic goals -drug-disease interactions -the necessity of dose adjustments Unfortunately, real patients don't always mirror the "textbook" patients. For that, pharmacists' involvement in some capacity is necessary. Unless it's a cash-based practice, fine-tuning NON-clinical skills in outpatient/virtual practices is a game changer for prescribers. An example of a non-clinical skill that can be fine-tuned is to know the best practice for prescribing certain medications. Being familiar with certain prescribing best-practices can improve a prescriber's workflow efficiency, decrease burnout rates, prevent delayed care for the patients e.tc.
