What's causing all this? Mental health can significantly impact maternal mortality rates in several ways: Access to Care: Women with mental health conditions may face challenges in accessing prenatal and postnatal care, leading to gaps in essential health services and increased risk during pregnancy and childbirth. Riskier Behaviors: Mental health disorders can be associated with behaviors that pose risks during pregnancy, such as substance abuse, smoking, or poor nutrition, which can contribute to maternal mortality. Medical Complications: Certain mental health conditions, such as depression and anxiety, are linked to an increased risk of medical complications during pregnancy, including pre-eclampsia, gestational diabetes, and premature birth, which can elevate maternal mortality rates. Postpartum Depression: Postpartum depression and other perinatal mood disorders can impact a mother's ability to care for herself and her newborn, potentially leading to adverse health outcomes and complications that contribute to maternal mortality. Stigma and Barriers: Stigma surrounding mental health can prevent women from seeking help or disclosing their symptoms, resulting in undiagnosed or untreated conditions that may escalate during pregnancy and childbirth. Addressing mental health within the context of maternal care is crucial to improving outcomes and reducing maternal mortality rates. This includes providing comprehensive mental health screening and support services, integrating mental health into prenatal care, reducing stigma, and ensuring access to mental health resources for pregnant and postpartum women. **Prevention of Obesity Begins in the Womb** We should consider shifting our focus to expectant mothers when discussing obesity prevention. In my clinical practice centered on obesity management, I have repeatedly encountered a concerning pattern, particularly among South Asian women. Many maintain a normal weight until their first pregnancy, after which they experience drastic weight gain—often up to 20 kg—followed by postpartum weight retention. With subsequent pregnancies, issues like gestational diabetes arise, leading to further weight retention. Autoimmune thyroiditis frequently complicates matters, resulting in additional weight gain and the onset of diabetes. It’s alarming to see how many South Asian women over the age of 35 are living with a diabetes diagnosis. While a sedentary lifestyle—common among South Asian women who are often not raised to be physically active—and a low-protein, high-glycemic index diet contribute significantly to this issue, an even greater factor is the widespread misconception that pregnant women must "eat for two." This is where prevention must begin: through education and proactive lifestyle management, particularly during the perinatal period. Many may not realize that excessive weight gain during pregnancy can have lasting effects not only on the mother but also on the child. Studies indicate that children born to mothers who were overweight during pregnancy are 2 to 3 times more likely to be obese by age 5.
