When a heart condition was keeping Laura Montoya, 39, from being active and doing what she is most passionate about, helping others, she knew something needed to change. While others suggested an invasive pacemaker, Laura was lead to Dr. John Clark, pediatric cardiologist at Akron Children’s. The type of postoperative care you need depends on the type of surgery you have, as well as your health history. It often includes pain management and wound care. Postoperative care begins immediately after surgery. It lasts for the duration of your hospital stay and may continue after you’ve been discharged. As part of your postoperative care, your healthcare provider should teach you about the potential side effects and complications of your procedure. Before you have surgery, ask your doctor what the postoperative care will involve. This will give you time to prepare beforehand. Your doctor may revise some of their instructions after your surgery, based on how your surgery went and how well you’re recovering. Prepare ahead of time Ask as many questions as possible before your surgery, and ask for updated instructions before you’re discharged from the hospital. Many hospitals provide written discharge instructions. Ask your doctor questions such as: How long will I be expected to remain in the hospital? Will I need any special supplies or medications when I go home? Will I need a caregiver or physical therapist when I go home? What side effects can I expect? What complications should I watch out for? What things should I do or avoid to support my recovery? When can I resume normal activity? The answers to these questions can help you prepare ahead of time. If you expect to need help from a caregiver, arrange for it before your surgery. It’s also important to learn how to prevent, recognize, and respond to possible complications. Depending on the type of surgery you have, there are many potential complications that can arise. For example, many surgeries put patients at risk of infection, bleeding at the surgical site, and blood clots caused by inactivity. Prolonged inactivity can also cause you to lose some of your muscle strength and develop respiratory complications. Ask your doctor for more information about the potential complications of your specific procedure. Postoperative care in the hospital After your surgery is complete, you will be moved to a recovery room. You’ll probably stay there for a couple of hours while you wake up from anesthesia. You’ll feel groggy when you wake up. Some people also feel nauseated. While you’re in the recovery room, staff will monitor your blood pressure, breathing, temperature, and pulse. They may ask you to take deep breaths to assess your lung function. They may check your surgical site for signs of bleeding or infection. They will also watch for signs of an allergic reaction. For many types of surgery, you will be placed under general anesthesia. Anesthesia can cause an allergic reaction in some people. Once you’re stable, you’ll be moved to a hospital room if you’re staying overnight.

When it comes to babies and toddlers, parents are taught to be very careful about small parts, due to the risk of choking. But another risk that has to be taken in consideration is aspiration, as in this case where little Asher was thankfully saved 🙏 FBA (Foreign Body Aspiration) is when food, drink or other material accidentally enters the airway tube and travels down to the lungs, instead of entering the food tube and travelling down to the stomach. This can cause serious health problems, such as pneumonia. On one side it allows more time for treatment, compared to choking incidents, but on the other hand the symptoms may be so "silent" to make it difficult to be detected (especially if parents are not aware of what happened, with infants not being able to explain), allowing the infection to grow severe before getting treatment. Be very careful about food and objects that can be very small and light, which increases the risk of being accidentally breathed in. Examples: popcorn, tiny marshmallows, small round chips such as the cheesy ones, powdered sugar, small polystyrene foam pieces... A good way to easily assess the risk of aspiration is to place the small parts on the palm of your hand and blow very gently. If they move very easily, better to keep them away from your small ones (or mitigate in your safety risk assessment, if likely to enter in contact with infants or toddlers) #productsafety #childrensafety #aspiration
In the blink of an eye, a fun arts and crafts activity turned life-threatening for 7-month-old Asher. Asher was watching others play with art sand when the granules accidentally got in his mouth as he inhaled, making it hard to breathe. Asher’s mom, Taya, called 911 and a fast-acting cascade of life-saving care began. First responders were able to get Asher breathing again and he was transported to his local hospital. More specialized care was needed to help save Asher, and he was transported to Children’s Mercy. When Asher arrived, our surgery, cardiology, blood bank, ECMO, ENT and pulmonology teams were ready to spring into action. Asher was put on an ECMO (extracorporeal membrane oxygenation) machine, which functions as the patient’s heart and lungs in life-threatening situations. Putting a child on ECMO is not an easy decision. But Children’s Mercy teams worked for hours to clear the sand out of his lungs – thankfully, with great success. “It was not a given that this child would survive,” said Dr. Sarah Brunner, Medical Director, Pediatric Care Unit Quality Improvement. “It took every single person at that bedside to make this outcome happen, and it had to be done fast. It was very much a team effort and provided a real connection to purpose and to our true mission.” Asher has now fully recovered, returned home and is back to being his smiley, happy self! 💙

The gender disparity in heart surgery outcomes is disappointing. Women are three to five years older than men when undergoing bypass surgery, in part because coronary artery disease is recognized more easily and earlier in men. As a result, women may have different symptoms, including fatigue, shortness of breath, and back or stomach pain. Furthermore, fewer than 20% of patients enrolled in clinical trials are female, so what we’ve been taught is based essentially on research in men. Women are also more likely to have developed health problems like diabetes, high blood pressure, and vascular conditions, which increase risk in cardiac surgery. They also have smaller, more fragile blood vessels, which can make surgery more complex. These disparities affect other forms of cardiac treatment and surgery, too. Cardiac Tamponade In EMS, chest pain and chest injuries are some of the most critical and time-sensitive patients we encounter. Cardiac Tamponade is no different. Cardiac tamponade is a buildup of fluid in the pericardial sac that surrounds the heart which impairs the heart’s ability to contract and the ventricles to fill normally. When the ventricles are not allowed to fill as they want, we see a decrease in stroke volume, a drop in cardiac output, and visible signs of shock. Cardiac tamponade often occurs as a result of trauma to the chest. Trauma can come from external, blunt, or penetrating forces to the chest wall itself, or it can come indirectly from things like surgery. Myocardial infarctions can also cause cardiac tamponade when they result in cardiac rupture. Further, there are different types of infections that can cause cardiac tamponade. Beck’s Triad: Low arterial blood pressure, distended neck veins, and muffled heart tones Treatment: 1- Apply high concentration oxygen 2- Consider C-spine and full spinal immobilization if suspected trauma 3- Check vital signs every 5 minutes Monitor ECG rhythms for any signs of arrhythmia 4- Ensure the patient is kept warm 5- Rapid transport to an appropriate facility 6- Establish IV access and administer fluids as needed Definitive Treatment: Pericardiocentesis is the definitive treatment that few EMS personnel can perform. When older adults in their 80’s & 90’s or older come to clinic with structural heart issues, the mission is clear. Help find solutions to enhance quality of life and preserve independence. Shortness of breath, leg swelling, abdominal fullness and swelling, fatigue & disruption of quality of life removing ability to remain independent. These are symptoms that are commonly part of the presentation of patients with a leaky tricuspid valve and truly severe regurgitation. In some cases, diuretics are helpful, but in many cases are ineffective, a short-lived solution, and have destructive side effects. It used to be a heartbreaking situation where we just told them there was no option other than a high or prohibitive risk surgery. Many would decline, appropriately so and clinically circle the drain. It was very frustrating and heart-breaking to say there’s nothing else to offer. But it was the reality. We would often just follow patients along because we hoped that over time newer therapies would become available. It makes it all the more amazing that we are on the cusp of tricuspid interventions becoming routine and mainstream whether through availability of commercially available devices or through clinical device trials. It has absolutely changed the game for many patients who simply had no other options prior and can now thrive with a meaningful quality of life. Challenges remain including expense, ability to tackle all forms of anatomy, treat larger valve sizes, & overcome imaging challenges and infrastructure that are very real limitations preventing scaling of these therapies. The fact is though the first big steps have been taken and the progress is exciting. Device time for both cases shown in the video was approximately 30 minutes only. On the left is the commercially available tricuspid valve clip (Abbott | Cardiovascular) and on the right is tricuspid valve replacement as part of an early feasibility process (Medtronic Cardiac and Vascular ). The clinical impact of reducing TR to trace in both these cases was remarkable on follow up, so many patients stand to benefit! The Top-Ranked Heart Hospitals in the Great Plains Region: Among the top 50 rated U.S. hospitals for adult cardiology care in 23-24, four were based in the seven-state Great Plains region. This included the Mayo Clinic, the 3rd highest-rated heart hospital in the country. In the most recent (23-24) U.S. News & World Report survey of American hospitals, over 770 hospitals were scored on their proficiency in providing adult heart care, including heart surgery. In this survey, hospitals were scored on assorted metrics related to their experience with adult heart patients, the scope of their adult heart care programs, and the degree of distinction a hospital has earned within the adult cardiology care community.
