Time to tie the knot and make a commitment to making primary care PRIMARY again? Shouldn't we be focusing on restoring and strengthening the relationship between the patient, their family caregiver and the primary care team? Healthcare based on a transaction, disconnected from primary care simply fuels our longstanding fragmentation syndrome and does not work. In the good old days, people looked to their family doctor, who was trusted and who knew everything about them and their health. The entire family was engaged in making decisions and following through with the care that needed to happen. So, do we long enough for the 'good old days' to do something to bring them back? When will we 'tie the knot' and 'recommit' our healthcare system to one that reconnects, restores and strengthens the trusted primary care, patient and family caregiver relationship so that continuity of care becomes the 'norm'...and with it better health outcomes? Isn't it becoming painfully clear that now is the time to work with the system of care that works...with primary care... and improve care coordination by engaging patients AND their family caregivers by giving them the tools they need to cinch the knot to directly connect them to their primary care team? This week, I was reminded that perception, especially a caregiver’s, is tinctured with complex emotions. Several healthcare professionals had informed the caregiver, “There is nothing left to do but hospice.” The caregiver voiced that he understood where their loved one was at in the life cycle yet kept asking for aggressive measures. He was struggling with the decision he was making regarding the direction of the patient’s care. What and how terminality was presented by the interacting care team was a big factor in the caregivers' understanding and perception. These are not one-and-done conversations. Dialogue has to be fluid and ongoing, exploring gaps and doing the best to help guide and foster shared decision-making.
