It's another Coffee & Contracts Tuesday!☕🍪 Here are this weeks 5 highlights from the breakout room I moderated on the intersection of #datasecurity, #compliance, and #qualityimprovement from a Community Care Hub perspective. 1) ITS A WHOLE LOTTA LOTTA WORK to stand up the technology infrastructure, governance and oversight infrastructure, and audit-passing processes for community social care organizations that are required by contracts with #managedcareplans and other #healthcare entities. Often, it takes years. 2) MEASURES ARE DIFFERENT. Health Plans work in CPT codes and ICD-9 codes. Community social care providers deal in Social Needs Categories. 3) THE TOPIC EVOKES STRONG SENTIMENTS ALL AROUND. Participants used words like "paralyzing fear" to describe how some health plans feel when trying to navigate partnerships with CBOs, and "beast" (my personal favorite) to describe the complex set of requirements that CBOs must navigate to do business with health plans. 4) GENUINE CONCERN FOR PATIENT PRIVACY. It's one thing to sign a BAA and agree about data sharing standards. It's another thing to feel actually and personally comfortable about sending sensitive data (e.g. behavioral health data) about clients you care for outside your organization when you are not in control of it once it leaves and you very well may not have IT training. There is a patient/client care vulnerability there for care providers that is real. And, thank goodness for it! 5) SIZE MATTERS. Building up the capacity for interoperable data systems or access into existing health care systems is one thing if you have 10,000+ employees and a budget to match. But, for small and mid-sized community social care providers, the struggle can be real. Requirements set by large, mature well-funded organizations can be very hard for smaller organizations invited to collaborate in new ways that are different from their decades-long history. Healthcare has always been centered around the patient (as it should be) but let’s not forget that the hospital is also a workplace. Improve finances to improve more and better delivery team members. This better practice environment will be more productive and will improve finances. The practice will also save on turnover, orientation, recruitmente, and retention costs and losses. The focus is specific to the only innovation that matters - the one on one innovation between delivery team member and patients. HRSA must tell CMS to let their people go - their community health centers. HRSA must tell CMS to stop voting their 1.4 trillion dollars against primary care, mental health, womens health, geriatrics, and care for most Americans most behind. CHCs are trying to make a differences where Americans most need care, but CMS defeats them. CMS also defeats any training intervention, including its own Graduate Medical Education dollars. Training interventions including pipelines and special training such as Teaching CHC residency programs, are only rearrangements of the deck chairs until there is substantial increases in funding - such as an annual upgrade to 400 billion. This is far beyond the 250 billion pittance for primary care which is actually even less due to more costs of delivery and more burdens placed on delivery team members. Our patients that are most vulnerable need our full attention. We should stand together and tell the cost cutting/obsessive measurement disorder/micromanagement from far away crowd that the only innovation that matters is one on one with each patient. And when there is better support, we can allow them some polishing activities - but after not until major funding changes.

It is Day 9 of Careers in Public Health (CIPH)'s challenge and I am super excited to have participated in this. Today, I completed the final module of my online course from OpenWHO on "Maintaining Action on Health-System Priorities while Supporting an Emergency Response." Balancing health-system priorities with emergency response has to do with including emergency plans into overall health strategies. Services like maternal health and chronic disease management must be maintained to prevent health deterioration. Also, clear communication within the health system and with the public is important for dissemination of accurate information. Other strategies such as leveraging digital tools, training healthcare workers for dual roles and allocating resources ensures both areas are well supported. These steps help manage emergencies effectively without compromising essential health priorities. Finally, maintaining the priorities of health system while addressing emergencies is not just possible but essential. It means being prepared, staying focused, and always putting people's health at the forefront of our efforts. #10dayscareerchallengewithCIPH #10daysofcareerdevelopment
