Medicare cuts, obstinate commercial payers, and rising operating costs are threatening the viability of independent private practices. Chris Kean of our partner TSAOG Orthopaedics & Spine astutely noted in her testimony before the US Congress this week, that the way forward has increasingly become to cut out the noise by direct contracting with employers. "Strategically building [an] ecosystem for our community that includes all orthopaedic services to include the ASCs has allowed us to offer a 'value proposition' to those seeking our services at a lower cost setting with better quality outcomes. In fact, the largest area of growth in contracting for our Group is working with employers directly." This is why we are building a platform to enable independent MSK practices to price, build, and scale direct-to-employer contracts. If virtual MSK companies can reach multi-billion-dollar valuations based on comprehensive non-surgical treatment and avoidance of procedures, why can't surgeons? We can -- if the proper mechanisms are put into place. MSK VBC has centered around surgical episodes of care which only reward measures directly tied to the procedure (lower readmissions, less post-acute care, fewer complications). They say nothing about whether the decision for surgery was appropriate or evidence-based guidelines were followed. The answer is condition-specific VBC and/or global subcapitation for specialty care. Implementing these models represents a significant change in the mindset of surgeons, payers, and employers. Care pathways must be carefully designed. Collecting and reporting data is paramount. Collaboration and transparency are critical. My time in Durham last week at Duke Med's Health Equity and Value-Based Care Summit providing a tantalizing glimpse into the future of MSK VBC. The lessons I learned are the focus of this week's "The Surgeon's Record" newsletter releasing tomorrow. Expect to invest several million dollars. 👥 Operational Differences: Cardiology cases demand experienced cath lab staff and a higher nurse-to-patient ratio for post-op care, emphasizing the importance of specialized training. 💸 Reimbursement Strategies: Utilize the new C-codes introduced in 2023 to maximize reimbursement, particularly for diagnostic and pacemaker procedures. This can significantly enhance your ASC's revenue. 🤝 Payer Negotiations: Start negotiations 9-12 months in advance and leverage physician data to secure favorable rates. Delay your go-live date if necessary to ensure optimal contracts. 🏥 De Novo Considerations: For new facilities, focus on volume and funding sources. Aim for at least 800 cases annually to ensure a quicker return on investment. Let's be honest, we're all kind of hearing about value-based care. The term means more or less whatever you want it to mean. No one can agree on what value is. The amount of talk far outweighs the amount of demonstrable results. I call it VBC Absurdism. That doesn't mean the idea doesn't have merit or that improved quality, cost, and accessibility aren't noble goals -- they are. We've just been going about it the wrong way (especially in MSK). Orthopedics has been on the forefront of VBC with programs like BPCI and CJR. CMS plans to sunset these programs within the next year, perhaps replaced by Making Care Primary and TEAM. Will these programs fare any better? Or is there another, better way. I think so -- as detailed in this week's The Surgeon's Record presented by Commons Clinic. We are thrilled to announce the publication of a new peer-reviewed study, conducted by Mid and South Essex NHS Foundation Trust in collaboration with B. Braun Group, and powered by MediShout technology. It’s been fantastic working closely with all of the teams involved in this study, particularly the MSE SSD staff, who work tirelessly in all they do. The research focuses on tackling the critical issue of surgical cancellations due to equipment failures through the digitisation of repair pathways. These cancellations place a significant burden on both patients and hospitals.
