PCHI is excited to share the findings of a recent study, spotlighting the efficacy of Pathways Community HUBs (PCHs) in advancing community-based care coordination. Our Chief Quality Officer, Amy Vreeland, was among the researchers involved in this study, which received support from both the Robert Wood Johnson Foundation and the Georgia Health Policy Center.
