Consider the Gap 🤔 After entering care, many individuals face a lack of follow-up care and continuous support, increasing the risk of relapse. What does “dying with dignity” mean to you? Do your loved ones have the same definition? How do you know? As a culture, we do everything we can to avoid talking about death… even when it’s on our doorstep. Conversations about Aging & End of Life–related wishes are difficult even for professionals working in palliative care. But when we skip over these conversations, then we miss a crucial chance to ensure that somebody’s final wishes can be known and honoured. As we enter #AdvanceCarePlanningMonth, we wanted to share this moving blog post from Dying With Dignity Canada. This story shows why it’s so important to have the hard conversations, while we still can.
