US-GUIDED SUPRASCAPULAR N BLOCK Patient seen recently for regenerative medicine treatment to the rot cuff. A suprascapular nerve block was first performed at the suprascapular notch to help with intra-operative and post-procedure pain. The challenges with blocking the nerve at the suprascapular notch are: 1. Finding the notch! Surprisingly, this is quite challenging for those who are relatively new to MSK US. 2. Not being in the notch, but advancing too far and hitting the apex of the lung. Not a good day! Make sure that you ALWAYS see the needle tip and that the needle is actually touching bone before injecting. Using a curved probe can be very helpful because it allows a steep injection angle yet still with good needle visualization. The advantage of targeting the nerve at the upper notch (vs. the inferior spinoglenoid notch) is that you get both branches-- supraspinatus and infraspinatus. Targeting the nerve at the inferior spinoglenoid notch oftentimes only gets the branch to the infraspinatus. Although stentgrafts have shown higher target lesion patency rates (TLPR) as compared to plain old balloon angioplasty (#POBA) for the treatment of cephalic arch stenosis (#CAS), edge stenosis is the Achilles heel. How would you treat this edge stenosis (14 months after placement) in a normal-flow single-outflow #RCAVF? Surely, extending the #stentgraft is a valid solution, but it will jail the axillary/subclavian vein and risk future access option. Cephalic turndown is another solution, yet more invasive than endovascular methods. I’ve opted for vessel preparation (#FLEX VP VentureMed Inc.), followed by POBA (#Mustang Boston Scientific) and #DCB (#INPACT Medtronic) with the hope to extend the #TLPR for as long as possible #VascularAccessCenter #Hamburg

ATLAS is intended for use in the treatment of stenotic or occlusive lesions of the iliac or femoropopliteal arteries to establish and maintain patency. The ATLAS self-expanding stent is indicated for use in percutaneous intravascular implantation in patients with atherosclerotic, radiation-induced and post-traumatic lesions causing vascular stenosis, hemodynamically significant and difficult to treat with other forms of treatment. #phlebologist #vascular
