Life-affirming physicians are committed to offering excellent healthcare equally to both pregnant women and their preborn children. Tragically, when treating life-threatening pregnancy complications, it will sometimes be necessary for the physician to separate mom from baby, even if the child is too young to survive outside the womb. However, the physician will never waver in his or her commitment to respecting the dignity of both patients. Our practice guidelines on concluding pregnancy state that, for a life-affirming physician, "It is acceptable to deliver a patient before the gestational age at which the fetus could survive outside the womb only if the mother’s life or health is in danger, which is proportional to the danger the fetus/neonate will face at birth. To be clear, this means the mother is facing death or immediate irreversible bodily harm which cannot be mitigated in any other way, including ectopic pregnancy and critical maternal illness." May 22 is World Preeclampsia Day, a good time to learn about this condition of pregnancy which is characterized by high blood pressure and can have short- and long-term implications for both the birth parent and baby, including on heart health. An inter-professional team of Sunnybrook care providers, led by the Department of Family and Community Medicine, is working to build awareness among patients and primary care providers on ways to mitigate the risks to cardiovascular health posed by preeclampsia and other hypertensive disorders of pregnancy. Sepsis screening tools designed to account for the physiological changes that may occur during pregnancy perform best from 20 weeks’ gestation through 3 days postpartum, suggests NICHD-supported research. In early pregnancy and later in the postpartum period, tools based on general sepsis screening criteria may more accurately predict which patients are at high risk for maternal sepsis, a leading cause of pregnancy-related deaths worldwide.
