Critical Control Verifications Thought I would share some insights on the implementation of Critical Controls. Critical Controls are robust barriers between identified threats, such as excavation in the vicinity of charged holes, and Material Unwanted Events (MUE’s) and the consequence thereof. Material Unwanted Events lead to catastrophic outcomes. In the drill and blast space we identify the following MUE’s · Unplanned detonation in operations · Unplanned detonation in storage and transport · Flyrock (surface) and personnel impacted by blast effects (underground) · Loss of explosives security · Persistent advert blast outcomes Resulting in the development of around 20 Critical Controls using Bow-Tie analysis. Over the past six years we have worked with a number of organisations and what we are seeing is a range of approaches to develop Critical Control and then a stumble on the Verification. The difference between a critical control and a procedural control aimed at managing a process is that a failure of a critical control needs to be identified and rectified within a defined (short) timeframe and trigger an immediate response. This requires ongoing verification at, typically, shorter timeframes than current compliance checks. Another factor to consider is the implementation of the verification schedule. Individual critical controls are required to be verified by different roles at different frequencies. For example, the Supervisor needs to verify the lightning monitor function at a higher frequency than the Superintendent verifies the veracity of the register of authorised persons. To address these issues, BMI has developed an integrated Critical Control Verification system based on a digital platform. Specific compliance checks to verify the controls are allocated to key roles at defined frequencies. The results of the verifications are analysed and reported to a dashboard which is customised and shared across the organisation. We are finding this to be an effective approach.
