Oral delivery of most atypical antipsychotics is challenged by their low bioavailability. “Most patients suffering from schizophrenia or bipolar disorder are prescribed a range of antipsychotic medications, which trigger significant adverse effects by disrupting the gut microbiome. . . . The most notable side effect is weight gain, with many patients often seeing increases of between 10-15% of their body weight after just three months of treatment,” said Paul Joyce, lead researcher, University of South Australia. However, antipsychotics can be reformulated to reduce weight gain and boost serotonin levels by more than 250%, according to Advanced Functional Materials. A study tested a new coating of tiny core-shell particles made from dietary fibre inulin and bioactive medium-chain triglycerides on lurasidone. This new formulation improved drug absorption 8-fold while overcoming weight gain. The inulin shell enhances the gut microbiome by providing an energy source for gut bacteria, while the medium-chain triglycerides facilitate drug absorption into the bloodstream ➡️ https://lnkd.in/dkkspvra. In liver disease, benzodiazepines that undergo minimal hepatic metabolism are generally considered safer. These drugs are less likely to accumulate and cause adverse effects such as sedation or cognitive impairment. ☑️Benzodiazepines that are metabolized primarily through conjugation rather than oxidation are preferred, as conjugation pathways tend to remain relatively intact in liver dysfunction. This includes temazepam, oxazepam, and lorazepam. ☑️This reduces the risk of prolonged drug action, toxicity, or further liver damage. 💊 Chronic Metformin Therapy and Vitamin B12 deficiency : ▪️Clinical Practice Guideline for Diabetes Management in CKD from @goKDIGO recommends annual monitoring patients for vitamin B12 deficiency when treated with metformin for > 4 years. ▪️Metformin can cause vitamin B12 deficiency by reducing the absorption of the IF complex through the enteral cubilin receptor in the terminal ileum, which can either cause peripheral neuropathy, cardiac autonomic neuropathy, neuropsychiatric symptoms, or hematological disorders ▪️Vitamin B12 deficiency, even without anemia, can mimic diabetic neuropathy, leading to misdiagnosis. Here is a publication of a peer-reviewed Phase 1 study of a novel intranasal formulation of #BPL003 by #BeckleyPsytech in The Journal of Psychopharmacology. The synthetic intranasal benzoate salt formulation of 5-MeO-DMT (also known as mebufotenin) was relatively well tolerated in combination with psychological support by 44 psychedelically unaffected healthy volunteers. BPL-003 showed a desired dose-proportional increase in pharmacokinetic -(PK) and pharmacodynamic (PD) effects. The short duration of action and the more than adequate results of the Mystical Experience Questionnaire (#MEQ30) as well as the correlation with the Ego Dissolution Inventory (#EDI) suggest a clinical potential that warrants further investigation.
