Failure to Follow Up I recently reviewed a case involving a failure to follow up on an outpatient CT scan that showed a “critical result.” It was ordered by the patient’s primary care provider for specific complaints the patient had. The patient then had two more encounters with this provider over the next two months with no mention of the scan or the results. A few months after the scan, the patient finally saw the specialist he was referred to before the scan was taken, who also never mentioned the CT scan or the results. Two providers over four months. It appears this scan completely fell through the cracks. This led to a delay in diagnosis and, ultimately, a delay in treatment for this patient. Here are a couple of thoughts I had: 💭 The ordering physician is ultimately responsible for following up with results from testing they order. This did not appear to happen. 💭 Usually, there is a critical reporting system in place for radiologists. When there is a “critical result” notated on a scan or a result that requires "urgent follow up", the radiologist will, in some shape or form, notify the ordering physician of those results. This did not appear to happen. 💭 Depending on the state, a notice of abnormal imaging results is required to be sent to the patient by the imaging center where it was completed informing the patient they need to follow up with the ordering provider. Was this the case? 💭 The specialist appeared to be in a completely different health system and did not have access to the scan through his EHR, so he did not know about it or the results as it was never communicated to him. However, with this data from RAND, you could feel more confident directionally speaking with an analysis if you just had access to facility fees to showcase savings to an employer and extrapolate from there.
