Last week’s TRICALS Masterclass, held in Amsterdam, was a huge success. With an engaged audience of about 120 participants representing a variety of industries, the Masterclass created an environment of free-flowing conversation with a variety of perspectives and opinions as well as thought-provoking questions Can we now consider Microfilaments as a possible primary endpoint in ALS studies? Should we now exclude placebo arms in early phase studies? What do we need from a Phase II study to confidently start a Phase III study? And what does a successful Phase III study look like? After two days of great discussion, here are some general take-home messages: 🟢 Clinical relevance should always take the patient’s perspective into account 🟢 Clinical studies should run for at least 6-9 months in Phase II and 12 months in Phase III 🟢 Good safety data, positive changes in Neurofilaments, and a positive change in a clinical marker (such as ALSFRS-R) should be available prior to starting a Phase III study 🟢 All Phase III controlled studies should include an Open Label Extension A big thank you to Professor Leonard van den Berg and the whole TRICALS team on a very successful, interactive, and informative #Masterclass. It’s been an exciting few days in Vienna at #UEGweek24 ! Having sat through some key data releases and symposia I’m more convinced than ever that practicing physicians don’t care about the data dump. For sure the data is important but what they really want is the “so what does this mean to me and my patients, what are the implications and how do I apply this in practice?” They count on reputable knowledgeable top-tier faculty that basically sign off the validation of the data and can provide practical and pragmatic guidance about the cutting science! P.S. Practicing physicians update their scientific knowledge online while scrolling, like you and me…. (and they don’t go back to first or second year medical school that is typically the first 2 presentations in a symposium!!), they want highly focused Microburst education in a clean simple no-distractions format. As I head out to ACR 2023 I reflect on the incredible progress made over the last 20 years. Remission is now a real target for treatment in RA, AS has been redefined and there are a great range of new therapies in PsA and incredible results in the skin. In Lupus new therapies have arrived and many more being developed with new outcome measures. It is a testament to a great collaboration between academics, industry and of course patients.
