Severe emphysema is a debilitating disease. Patients with severe COPD characterized by emphysema and hyperinflation may, however, be eligible for bronchoscopic lung volume reduction therapy with one-way endobronchial valves (EBV therapy), an established treatment for many years now. This review proposes a management strategy for when there is poor response to EBV therapy and is thus a must read for all users and those who are interested in establishing an EBV service at their institutions.
