This event drew together over 400 fertility specialists from across the region, emphasizing its prominence within the field 👩🔬 👩⚕️ Our CEO Hans Atli Dahl presented "Non-invasive PGT-A: Potential and Promise" focusing on the latest non-invasive preimplantation genetic testing for aneuploidies (#niPGT-A), a topic of significant interest to the attendees. Evolution of Genetic Testing in Reproductive Medicine In a recent podcast, I met with Dr. Joe Leigh Simpson, a distinguished figure in Reproductive Medicine, Obstetrics, Gynecology, and Genetics, discussed the transformative journey of in vitro fertilization (IVF) and the groundbreaking advancements in genetic testing that have revolutionized reproductive care. Dr. Simpson, with a prolific career marked by over 800 articles and leadership roles at prestigious universities, highlighted the significant shift from traditional protein-based screening to DNA-based analysis. The conversation delved into the Human Genome Project's role in decoding the intricacies of our DNA and its impact on genetic diagnosis. The discussion began with a historical perspective on prenatal genetic diagnosis, emphasizing the challenges before the Human Genome Project. Dr. Simpson recalled the pioneering techniques of amniocentesis and chorionic villus sampling, which allowed the examination of fetal cells for genetic abnormalities. However, the real game-changer emerged with the completion of the Human Genome Project, heralding an era where entire genomes could be sequenced efficiently. Dr. Simpson explained that, with advancements in technology, screening for thousands of genes became as economically feasible as targeting specific disorders. The conversation then shifted to the current capabilities of genetic testing, where neonatal blood samples can be rapidly analyzed to identify genetic mutations. This modern approach enables quick diagnoses for infants with severe medical issues, offering insights into potential genetic causes. Dr. Simpson emphasized that this comprehensive screening is not limited to specific disorders but covers the entire genome, providing a holistic view of potential genetic issues. This progress allows couples to seek genetic testing before conception, not only for fertility concerns but to mitigate the risk of passing on severe genetic conditions to their offspring. The podcast underscored the profound impact of genetic testing on reproductive medicine, transcending traditional infertility treatments. Couples can now make informed decisions about their family planning journey, focusing not only on conception but also on ensuring the health and well-being of future generations. To transfer or not to transfer? The dilemma of mosaic embryos is discussed in this new narrative review by Elkin Muñoz, Fernando Bronet, Belen Lledó, Gabriela Palacios Verdu, Lorena Martinez-Rocca, Signe Altmäe, and Josep Pla, representing the Special Interest Group in Reproductive Genetics of the Spanish Society of Fertility. The review summarizes current knowledge about mosaic embryos in the field of assisted reproduction techniques, and is intended to serve as reference material for practitioners in reproductive medicine who must manage a mosaic embryo result after preimplantation genetic testing for aneuploidies. Based on the assessment of current evidence in this paper, the authors suggest that embryos with a result within the mosaic range should not systematically be discarded or disregarded for transfer as this practice could negatively affect the cumulative live birth rate per cycle.
