Multidirectional instability is a challenging diagnosis. In many cases we try to use non operative treatment for as long as possible but as we know this doesn't always work. When patients have failed non op treatment we need to consider advanced techniques to give these patients a reasonable outcome after surgery. With the addition of the Arthrex knotless fibertaks we can preform a robust pan capsular plication that is low profile with good results. Check out this NEW technique with Seth L. Sherman, MD (Redwood City, CA), demonstrating an all-suture, retensionable technique for quadriceps tendon repair using Knee FiberTak® Button anchors and SutureTape. The Knee FiberTak Button anchors are a game changer for the treatment of extensor mechanism injuries! These new anchors allow the surgeon to suture the tendon with any preferred stitch configuration and suture type (SutureTape, FiberWire, etc) and then shuttle these repair sutures through the anchor. The construct can then be provisionally tensioned and retensioned after going through a range of motion. As the latest data continues to shift the paradigm from resection to repair regarding horizontal cleavage tears (HCTs) in the meniscus, the optimal repair technique to treat HCTs is still up for debate. Considering the criteria of healing for HCTs, two techniques have become increasingly popular among the orthopedic community. NEW TECHNIQUE! This one is a game changer for extensor mechanism injuries. In this technique Wiemi Douoguih, MD (Washington, DC), demonstrates a knotless, retensionable technique for quadriceps tendon repair using Double Knotless Knee FiberTak® anchors. This allows the surgeon to create a hybrid-type repair construct with traditional transosseous tunnels combined with knotless, tensionable anchors. Total knee revision, Flexion gap first technique. I found it very effective in a recent case, keeping things simple. https://lnkd.in/emRKRRJt This is a great technique, as also described by Kelly G. Vince and Martin Bedard in the *Master Techniques in Orthopaedic Surgery: Knee Arthroplasty, Third Edition*. With the improvement of the LCCK NexGen to the Persona revision, it seems that revisions are more often based on the extension gap first? This involves adjusting the femur size if needed to + size to add more posterior condylar offset while keeping the same medial-lateral sizeq, according to what I’ve seen in some HSS educational videos.
