Actually, books goes to say about 5 elements, so called SHELL model with “ L “ in the meddle . Brake Interaption with one of the elements lead to the accidents and incidents to happen. Good example ilusttation on the picture . Pilot who is in the centre “ L “ Liveware is reading checklist “ S “ software and cheking out instruments in the cockpit “ H “ Hardware . Assuming , pilot not reading checklist or reading passively , not checking out switches and lights. What can happen we know. For me, it is very good tool to stay SAFE !
