With OTAs just starting, football is on the minds of many. As a proud to be born and raised in the City of Cleveland and die-hard Cleveland Browns fan, I've seen firsthand the impact of throwing injuries on our quarterbacks. Deshaun Watson's recent recovery from a glenoid (shoulder) fracture is a powerful reminder of the importance of injury prevention and management. Watson is on the road to recovery partly due to the consultation of leading specialist Neal ElAttrache, a former American Orthopaedic Society for Sports Medicine Board President. Just like Watson, many young athletes face the risk of throwing injuries across various sports, not just baseball. Whether your child is into football, lacrosse, or any other sport, these injuries can have long-term impacts if not properly addressed. Joint manipulation in sports medicine 🏀 While I am not a big proponent of joint manipulation as a modality, there is certainly a place for it within sports medicine. One of the things I learned from Dan McClure was the concept of ‘small bones’. This concept applies to the hands and feet where many bones and joints must move without restriction for optimal function. In basketball, I have seen primarily lack of medial rotation of the cuboid which may lead to arch pain due to the excessive lateral rotation and ‘drop’. 🔑 proper motion of the whole foot allows for good compression/expansion mechanisms when running and jumping. This is my version of the cuboid whip. Youth athletes return to sports at a high rate (91%) after ACL reconstruction. Unfortunately, the prevalence of a second ACL injury is high at 32%. Within this population, an earlier return to sport is predictive of a second ACL injury. Young athletes who do not achieve a 90% score on a battery of tests that measure fitness to return to athletic competition following anterior cruciate ligament (ACL) reconstruction are at increased risk for a second knee injury, according to research presented at the 2019 Annual Meeting of the American Orthopedic Society of Sports Medicine. The need for testing of knee stability, strength, neuromuscular control, agility, and psychological measures before RTS remains paramount in young athletes. The recent data that relates concussion to an increased risk of musculoskeletal injury is in line with prior studies that point to neurologic risk factors in anterior cruciate ligament (ACL) injuries. Of particular relevance, noncontact ACL injuries have been associated with lower neurocognitive function in non-concussed athletes, which suggested that neurocognitive differences may be associated with the loss of neuromuscular control and coordination errors leading to increased risk of ACL injury.

Elbow pain in throwing athlete is not so simple as many think! It has been a while since I've met a baseball pitcher with an elbow pain, but recently had the privilege to help a high level baseball pitcher with a medial elbow pain which made me dive deeper into the researching possible differential diagnoses and the evidence based management of elbow pain in throwing athletes.
