Effectiveness of an Eight-Week Multidisciplinary Selective Mutism Treatment Group: Recent selective mutism (SM) treatment approaches focus on the delivery of interventions using intensive doses of cognitive behavioral therapy (CBT) and Parent-Child Interaction Therapy (PCIT-SM). In the current study, we sought to examine the effectiveness of an eight-session weekly outpatient group program for youth ages 3 to 14 diagnosed with SM. Group interventions included caregiver coaching and support for CBT skills, particularly graded speaking exposures for youth using the PCIT-SM framework. Is online therapy as effective as in-person therapy? YES! A 2020 study, “Psychotherapy at a Distance,” found that online video therapy, primarily for cognitive behavioural therapy (CBT), has been as effective as in-person therapy. The study found that “remote therapy grants the therapist revealing glimpses of a patient’s home and life.” Interactions with children, pets, roommates and spouses that can be heard or witnessed while the individual is in their own personal space will be more genuine glimpses into that person’s world. Good luck 🍀 to Dr Sharon Collins and Sai Achuthan from Cygnet Kewstoke today, who are presenting their QI project titled "Introduction of the Diamond framework, creating a psychologically informed, cohesive environment" at the 2024 Annual The Royal College of Psychiatrists QI conference in London. Cygnet Kewstoke were facing a significant increase in self-harm incidents among their Service Users on Knightstone Ward, a Personality Disorder service. This was coupled with poor attendance in DBT and OT therapy sessions, hindering SU progress and recovery. Additionally, there was a noticeable split among the staff, including the MDT and nursing team, which affected the consistency and quality of care provided to SUs; notably, some staff were known to be having lengthy 1:1 sessions with them, which ultimately led to ‘competition’ for staff support and decreased willingness to self-regulate. These issues highlighted the need for reviewing the existing processes and developing a unified, evidence-based approach to create a more psychologically informed environment and improve patient outcomes. The SMART aim set for the project was “To reduce self-harm incidents by 25% on Knightstone Ward by March 24 through the introduction of the Diamond framework, creating a psychologically informed environment.” The incredible key outcomes of this project are shared within the graphic.
