What happens if you don't respect anatomy at the time of total hysterectomy? ❗ Do not underestimate the risk of prolapse. ❗ Respect the anatomy and attempt to restore it after removing the uterus even if there is no obvious prolapse present. ❗ Even in our day there are still people "treating" uterine prolapse with simple hysterectomy. ✳ Remember, the cervix is the anchoring point of the pubocervical fascia, the cardinal ligaments and the uterosacral ligaments as an extention of the rectovaginal fascia. If these structures don't reattach after removal of the cervix, the pelvic floor is bound to fail like an arch without its keystone. #urogynecology #praktischeurogynäkologie #gynaecologia #gynecology #urology

⚠ Case of the week! Control examination 6 weeks after TVT placement. With routine pelvic floor sonography we are used to check: 1. 𝐓𝐚𝐩𝐞 𝐝𝐢𝐬𝐭𝐚𝐧𝐜𝐞 𝐟𝐫𝐨𝐦 𝐛𝐥𝐚𝐝𝐝𝐞𝐫 𝐧𝐞𝐜𝐤 (aim to be at 50-70% total urethral length), 2. 𝐓𝐚𝐩𝐞 𝐝𝐢𝐬𝐭𝐚𝐧𝐜𝐞 𝐟𝐫𝐨𝐦 𝐥𝐮𝐦𝐞𝐧 (aim to be about 3-5mm but even more may be acceptable in cases of hypermobile urethra) and 3. 𝐓𝐚𝐩𝐞 𝐟𝐨𝐫𝐦 (straight in relaxation and C during Valsalva). All of the above are typically done in B-mode. What about 𝐭𝐚𝐩𝐞 𝐢𝐧 3-𝐝𝐢𝐦𝐞𝐧𝐬𝐢𝐨𝐧𝐚𝐥 𝐬𝐩𝐚𝐜𝐞? Well, do have a look! ⚠ Case of the week! There has been a lot of criticism against vaginal meshes (and with good reason). In this small clip, you see how an anterior mesh may hold the bladder back (no cystocele). However, this patient eventually came to see us, because she still has to deal with a serious enterocele and a small rectocele. [PCNL/ECIRS - Position 5 - Prone split leg position] After knowing the different positioning, can we actually perform prone ECIRS? Yes, indeed! Back in 1993, Dr Michael Grasso (together with Dr Richard Nord and Dr Demetrius Bagley) described the prone split leg position for simultaneous antegrade and retrograde access to upper urinary tract. They also described the difficulty in performing ECIRS - two skilled endoscopists, dual light sources, various lithotrites, and several rigid and flexible endoscopes - that echoes the issues that hinder the development of ECIRS.
