End of this journey. Am officially completing the project after almost 1 month and half spending time and accepting the challenges during this process. Since been offered to assign to this project I know what kind of preparation and execution I need to do and fair enough I do have very great support system from team either yard St marine, vessel crews or Seaway 7 operation team. No single incident or unplanned events happened due to collectively great safety mindset and safety culture brought to the table by everyone. There were multiple times when challenges received from daily ST Marine teams, but from me to them were continous intervention, educating, non stop monitoring at site and way out discussion to enhance their ability to work together safely. Vessel is sailing out today as well to her location and all the best to her and team, i will see you when the time is coming soon. Thanks to St Marine team for producing this video since beginning until now. All load test, function test, heavy lifting, numerous welding drills grinding and other hot works not to forget Loto were happening during this framework were done in the safe manner and I really enjoyed it. How to assist in this process was to have multiple times of HIRA, Risk assessments and job hazards analysis on daily basis. All the best to everyone involved and thank you for the support. IOGP Well Control Incident lesson sharing 24-9: Retrieval of lower completion results in WCI. During retrieval of a lower completion an influx was swabbed into the well. There was no possibility to circulate below swellable packers. In your review, please consider what failures in your operations would lead to implementation of contingency procedures. IADC ISP Report 2023 Fatalities: In 2023 the industry’s fatalities decreased to 10 incidents compared to 15 incidents reported in 2022. According to the data reported in 2023, three (3) fatalities occurred to employees who had between five and ten years of service with the company. Three (3) more fatalities occurred to employees who had less than five years of service. Three (3) fatalities occurred during routine drilling operations. Four (4) of the fatalities occurred to floormen. Greatest Risks: Incidents occur in many places around the rig and to all crew members. Many types of equipment and rig operations are involved in incidents and all parts of the body are at risk. Incident data are analyzed by occupation, body part, incident type, equipment type, operation, location time in service and time of day the incident occurred. Data from the IADC ISP show that of all the categories, as with previous years, the Floorman position incurred the largest percentage of Lost Time and Recordable injuries. *Fingers remained the most vulnerable part of the body. *Caught Between and Struck By accounted for the most Lost Time Injuries and Total Recordable injuries. *Pipes/Collars/Tubulars/Casing is the equipment responsible for the most Lost Time and Recordable incidents. *Tripping In/Out and Rigging Up/Down accounted for the most Lost Time and Recordable incidents. *The most Lost Time and Recordable injuries in drilling operations occurred on the rig floor. *Lost Time and Recordable injuries based on Time in Service were led by employees with between one to five years of service. *The Most LTI and Recordable incidents occurred between 09:00-17:00 hours. *March accounted for the most Lost Time and Recordable incidents. *The 26 – 35 year age range accounted for the most Lost Time and Recordable incidents. Conclusion: Activity in most sectors of the industry increased in 2023. Overall, Lost Time incidents rates and Recordable incident rates decreased slightly in 2023 as compared to 2022, despite an overall uptick in rig activity. **USA Stats improved only because probably rig count fell off a cliff. -20% from a year before. According to the International Labour Organisation, shipbreaking is among the most dangerous jobs in the world. If not done properly it can lead to unacceptable safety, environmental and hazardous waste issues. It most often happens in developing countries and is linked with an unacceptably high level of fatalities, injuries and work-related diseases. That is why IOGP believes it is important to outline a set of guiding principles for the safe and responsible decommissioning and demolition of FPSOs for our industry. We have set out ten guiding principles, built on a foundation of regulatory compliance, which, together with any necessary class and flag compliance requirements, enable safe and responsible FPSO decommissioning.
