ISMP's Targeted Medication Safety Best Practices for Community Pharmacy #3a- Use a weekly dosage regimen default for oral methotrexate in electronic systems when medication orders are entered. #3b- Require verification and entry of an appropriate oncologic indication in order entry systems for daily orders. #3c-Create a forcing function (e.g., electronic stop in the sales register that requires intervention and acknowledgment by a pharmacist) to ensure that every oral methotrexate prescription is reviewed with the patient or a family member when a prescription is presented or refills are processed. #3d-Provide specific patient and/or family education for all oral methotrexate prescriptions. #ISMP #medicationsafety #pharmacovigilance

Just attended an insightful webinar from the ISMP (Institute for Safe Medication Practices) on their new targeted medication safety best practices for community pharmacy. Among the best practices are: #1- To use a standard protocol to verify a patient's identity, utilizing at least TWO patient identifiers (typically full name and DOB), when receiving a prescription to be filled, responding to patient-specific questions, providing filled prescriptions to patients at the point-of-sale, when delivering prescriptions to the patient's home, and prior to administering vaccines or other treatments. https://lnkd.in/eb-Q4h4x #ISMP #medicationsafety #pharmacovigilance

ISMP's Targeted Medication Safety Best Practices for Community Pharmacy #4- Standardize to the use of milliliter (mL) unit of measure when prescribing, dispensing, and measuring oral liquid medications. - Eliminate the use of "teaspoonful", "tablespoonful", and other non-metric units - Purchase and dispense metric-only liquid dosing devices - Dispense a dosing device that most closely matches the prescribed dose volume needed to administer one dose - Educate patients using the teach-back method #ISMP #medicationsafety #pharmacovigilance

After the International Pharmaceutical 3D printing Initiative (Pharma3DPI) online conference last month, seeing another similar event by CurifyLabs and observing how fast things are developing in the field made me optimistic about the future of medicine. Such events pique my interests, especially when they turn out fruitful. In response to one of the key takeaways mentioned, I believe, dedicated educational materials should soon become part of the curriculum of professional programs like MD and PharmD degrees, among others. This would foster and ease collaboration among clinicians, pharmacists, and other professionals in related fields who can then easily find common and familiar ground. Importantly, it would also help garner greater recognition for the field among younger generations, ideally attracting more enthusiastic and innovative minds. #personalizedmedicine #3Dprinting #Percisionmedicine #Pharma
