A recent report from the Centers for Disease Control and Prevention found #rural Americans were more likely to die from heart disease, stroke, unintentional injuries, cancer and chronic lower respiratory disease before 80 years old, compared to those who lived in urban communities. At Homeward, we support rural communities through a combination of local provider partnerships, referrals to local resources and social services, technology, and Homeward’s own clinical services. Through partnership and collaboration, we can best understand the unique health, social and environmental inequities rural Americans face and create effective interventions that reduce risks and improve health outcomes.
