No surprise here. If this country wants to get serious about training and maintaining a vital Primary Care workforce, it needs to value and compensate Primary Care visits significantly more. This is in addition to an overhaul of our current third-party and employer-based insurance model for financing health care and tort reforms needed to slow down the seemingly inexorable increases in health care costs in this country. The stresses of having to maintain a broad-base of medical knowledge amid rapid medical advances including a growing diversity of drugs, devices, and best practices statements, needed by the Primary Care Provider combined with lower compensation is a sure way to encourage further migration of future Physicians and Advanced Practice nurses and Physician Associates away from Primary Care and towards a narrower scope of practice and the more financially lucrative specialty care fields.

Doctors Medical Center of Modesto, California, has received an immediate jeopardy citation from the Centers for Medicare & Medicaid Services (CMS) due to issues with certified registered nurse anesthetist (CRNA) credentialing. The California Department of Public Health (CDPH) confirmed that the Tenet Healthcare-owned hospital allowed CRNAs to oversee procedures they had not previously performed there. Following this, an internal memo indicated a corrective action plan was submitted, and CRNAs are barred from returning until CMS approves this plan. The 394-bed facility has faced scrutiny before, with deficiencies noted in 2022 during federal inspections. CDPH teams are currently on-site to investigate compliance with state and federal regulations. The hospital and CMS are working to ensure adherence to requirements to provide safe, high-quality care. Further information will be available upon the completion of these investigations. Becker's is monitoring for updates and has sought comments from CMS and Doctors Medical Center. Follow the money! Then, follow the job satisfaction numbers... If we want more bright young doctors to choose primary care, we need to modernize the compensation and the job. I've begun advising those in primary care residencies to spend a year or two in a traditional practice and then to seek direct primary care or concierge opportunities. Hospital medicine is also an option. In many health systems, primary care doctors have become glorified referral clerks. The job is not fulfilling when there is no time and no respect. It's not a mystery why most medical students will tell you they want a residency in anything but primary care. The primary care physician shortage is 13,000 and growing. In ten years it may be 40,000 providers. More to come on this topic...
