ASCP visited a skilled care nursing home in Warsaw. 5 buildings on a campus (1,000 beds) with an onsite pharmacy staffed by 4 pharmacists. They are doing a technology pilot but ALL medications in Europe are manufactured and dispensed in unit dose packages in boxes. They are “punched” by nurses into soufflé cups. Even the new technology uses soufflé cups in a “pick to light” method. Shouting from the rooftops: Providers please stop prescribing insulin to patients without 1:1 education on how to administer it, explaining signs and symptoms of hypoglycemia, and a prescription for Gvoke or Baqsimi! We are in such a hurry to remove the RNs from physician offices and replace them with lower cost medical assistants that we are forgetting the patient at the forefront! RNs are educated in assessing patients needs, identifying SDoH (“I can’t afford to fill this medication,” or “I don’t understand what it’s for?”) providing education on injectable administration, side effects, resources, etc. If providers are going to replace the registered nurse in the office then the responsibility of assessment of SDoH and education falls on the prescriber, not the unlicensed staff. We are setting patients up to fail. When they return in three months with an A1c that hasn’t changed they are being shamed and stigmatized into believing it’s their fault when they were never given an opportunity to succeed.
