ESFI recently surveyed EV installers to gauge their understanding of the safe installation of electric vehicle supply equipment (EVSE). When asked about EVSE training, 76% of installers said they had specific training, while 82% believed EVSE should have a specific certification. Some very interesting data from one of my colleagues in ADI's automotive functional safety group. Over 4000 automotive recalls analyzed. Almost 40% of the failures due to software, another 30% due to cables/connectors, 7% due to production and installation leaving 25% for hardware failures but with only 1% of those assigned to IC failures. I imagine industrial applications can't be that much different but I have never seen such detailed data.

The initial report indicates an improper handover / communication during the normalization of the MED/Plug (MID EXIT DOOR ) after post-rectification of rivet replacement on the assembly line. This may be attributed to multiple stakeholders' involvement, and it appears that both the OEM quality system and duplicate inspection failed to address these issues. Take-home note for all maintenance crews is to maintain detailed maintenance records with step-by-step phases and progressive signatures for all major maintenance tasks ensuring strict adherence to manuals or OEM instructions can avoid such unfortunate incidents..

2moIt should be mandatory for anyone working on the vehicles. I've had to warn a few young techs not to touch or use non-hybrid insulated tools around the batteries.
