‘Retreads’ is one of my favourite metaphors. It aptly describes the use of ineffective and inherently weak corrective action following an incident. I was gifted the Retreading metaphor by my friend Alan Smith many years ago. It’s a descriptive metaphor that represents the collective practice of using quick, simple but broadly ineffective fixes following an incident. Retreads are common after safety, quality and operational incidents/investigations. The metaphor refers to the practice of retreading vehicle tires, rather than replacing them. Practices that could be considered retreads include: ▶️ Re-communicating information what has already been communicated. ▶️ Re-training. If the learning did not work the first time… ▶️ Reminding. Surely the weakest of all corrective actions. Also Re-stating. ▶️ Reinforcing. What more could be gained by focusing on why a particular form of behaviour or action is occurring? ▶️ Reviewing. Too often reviews of policies and programs do not lead to concreate changes. The opposite of Retreads is re-engineering – fundamentally changing the context, conditions, environment or work set-up i.e. higher in the hierarchy of control. I was reminded of Retreads today while reading Ben Hutchinson's review of a study of corretive actions from incident which can be accessed here: https://lnkd.in/eFzYCDr4 I find Retreads a great example of the power of metaphors to explain a concept or represent an idea. Active errors are immediate errors caused by individuals, while latent errors are organizational errors that can exist within the system for prolonged periods of time going unnoticed until acted upon. Dividing human errors into active and latent categories is key in high-risk industries. I'm guessing that when closing out #investigation reports 🔎, most will always include an action such as 'Circulate lessons learned to.....' Sound familiar? Of course it does. We send the email and boom! - first action closed out❗ But maybe we need a terminology update? Because if we're being honest with ourselves, we only learn the lesson when we've done something to stop the same thing happening again. Unless we're sure that the other actions we've put forward have been adopted, embedded and are actually working, we've really no business sharing 'lessons learned'. I'm all for sharing #lessonslearned when the lesson has actually been learned. But if you're sending it out 📨 before the implementation of the actions, probably best to just call it what it is: 'Hindsight Bulletin'. Learn the lesson, THEN share the message! #improvement #safety #rca
