New free online course with Eric Plakun. All clinicians face the problem of working with patients whose treatments are not going well and who are not making progress. Clinicians often label these patients as "difficult" or "treatment resistant." This presentation addresses clinicians' own contributions to this experience, including the role of reductionist biomedical perspectives that do not match evolving evidence. The presentation also summarizes psychodynamic principles that may improve outcomes and the role of intermediate levels of care in treating some of these patients. https://lnkd.in/eqD6Xme6

"Evidence continues to mount for the benefit of looking beyond diagnosis and symptoms in psychotherapeutic treatment. First Caspi discovered that only one transdiagnostic 'psychopathology factor' underlies disorders, and then Jeppesen, among others, noted that trans-symptomatic treatments not directly targeting symptoms work as well as those that do [see this CE/CME course - https://lnkd.in/ehM3SGd3]. Now this 147-study meta-analysis from European investigators suggests that the same is true for suicide. Therapies seem to work equally well in reducing the risk of suicide whether they do or do not target suicide directly. It is this kind of transdiagnostic, trans-symptomatic perspective that underlies the development of our Alliance Based Intervention for Suicide [ABIS]. Rather than being a stand-alone symptom focused therapy, ABIS is a way to help psychodynamic therapists establish and maintain an alliance with suicidal patients [see this CE/CME course - https://lnkd.in/ezAVJKQy]. Across both the residential and IOP programs at Austen Riggs, suicide is addressed in treatment with considerations of the role of community, development, and nurturing a sense of identity and agency in the world in a way that promotes resilience and connection.
