The ReproLife team visited Kobanawa Clinic and conducted an interview regarding the “Freeze All” method. As oocytes age over time, the success rate of fertilization decreases with each oocyte retrieval. To prevent this aging of oocytes and to avoid the potential risk of ovarian hyperstimulation syndrome (OHSS) caused by the use of ovulation-inducing drugs, the “Freeze All” method allows for the preservation of oocytes while they are still young. Vesicoureteral reflux vs overactive bladder I recently assessed an 8 year old girl who presented with urinary trouble. Her mother explained that the patient was having dribbling of her urine throughout the day. She was convinced the patient had an VUR. There was no history of accidents, bedwetting and her ultrasound was normal. I used some good ol’ A&G and got a response from paediatric urology. It was to consider overactive bladder as the possible diagnosis. We tend to think of overactive bladder in older people but it is common in children too. So how do we treat a child’s overactive bladder? Think of the causes: 1. Is the patient constipated? 2. Is the patient not drinking enough? Concentrated urine can irritate the bladder lining. 3. What are they drinking? Fizzy drinks, caffeine or chocolate drinks can irritate the bladder 4. Are they holding on for too long instead of going to the toilet? 5. Is there a UTI? Don’t forget your basics of doing a urine dip! The trick here is to be comprehensive and not miss out the simpler, lifestyle causes of bladder irritation.

Postpartum hemorrhage (PPH) directly impacts the associated hospital cost. A recent article by NewYork-Presbyterian Hospital and Columbia University Irving Medical Center published in the American Journal of Obstetrics and Gynecology highlights the costs associated with postpartum hemorrhage based on levels of severity. Noted within this article is the rise in costs as hemorrhage progresses to more severe levels given the need for blood products, medications, medical devices, etc. In addition, the most severe and costly cases of hemorrhage occurred more frequently during C-sections. Understanding these costs is crucial for hospitals to prioritize investments in products that can not only reduce complications for mothers, but also reduce costs. Prevention of even just a handful of severe cases of hemorrhage provides an opportunity for hospitals to improve quality measures as well as reduce costs. Severe stress causes an increase in biological age, while recovery tends to reverse it, according to a study published in the journal Cell Metabolism. Unlike your chronological age, which is determined by your birth date, your biological age can be impacted by disease, drug treatment, lifestyle changes, and environmental exposures, among other factors, according to researchers. Other temporary changes in biological age occurred during instances of major surgery, pregnancy, and severe COVID-19 in humans or mice. The study found that trauma patients experienced a “strong and rapid” increase in biological age following emergency surgery; however, the increase was reversed and biological age returned to baseline following the recovery period after surgery. A 78-year-old woman presented to the emergency department with acute low back pain radiating to the groin and the right lower limb. Due to a history of chronic right lumbar sciatica, she was diagnosed with an exacerbation of lumbar sciatica and referred for intermediate care for pain management and rehabilitation. Upon my assessment, the patient exhibited abdominal pain and right lumbar sciatica with sensory-motor signs that responded very poorly to analgesic treatment. During the POCUS, I observed significant bladder distension and grade I-II right hydronephrosis. Laboratory tests revealed leukocytosis, elevated CRP, and acute renal failure with a BUN/creatinine ratio < 10. I indicated bladder catheterization, which resulted in the discharge of pus. I suspect acute pyelonephritis and initiated empirical antibiotic treatment, pending the results of the urine culture, with progressively improved control of lumbar pain. Due to the emergence of motor symptoms and acute urinary retention, she also underwent a follow-up MRI, which described worsening of multiple disc protrusions and ruled out spondylodiscitis.
