Earlier today, I had the fantastic opportunity to present at Safer Care Victoria’s Learning Lab session alongside Alexandra Kemble (middle of photo) ,Acting Short Stay/DOSA NUM, and Jesusette Mabutin (right in photo) , Surgical Discharge Coordinator. We shared our journey implementing Criteria Led Discharge (CLD) at Peter MacCallum Cancer Centre aiming to transform patient discharge processes. We are still in the early stages of our implementation, continually learning and adapting to ensure the best outcomes for our patients and staff. We are hoping that CLD will: • Improve patient experience by providing information on the milestones they need to achieve before discharge, which can help reduce anxiety and increase satisfaction with their care. • Improve staff experience through streamlined workflows, reduced conflicts, and increased job satisfaction. • Contribute to wider Healthcare System benefits from increased bed availability, cost efficiencies, improved patient flow, and reduced readmissions. After months in ICUs at some of the world's best medical centers my condition remained a mystery. However when I was transferred to Little Rock, Arkansas, known as the Castleman disease epicenter my bus driver recognized the signs of my condition with just a glance. I think this story highlights a crucial aspect of healthcare: the importance of specialized centers especially for rare disease patients. It demonstrates how concentrated expertise and community familiarity with specific conditions can lead to insights that might elude even the most prestigious general medical institutions. It begs the question: How can we leverage and share the deep, specialized knowledge found in certain communities and centers with the broader medical world?
