Interventional nephrologist Dr. Neghae Mawla returns to the show to discuss endovascular AV fistula creation with our host Dr. Christopher Beck. Patients who come to see Neghae receive a standard vein mapping via ultrasound to determine whether they should receive an endovascular or a surgical procedure. If patients’ veins fit certain specifications, such as superficial location (cephalic, median cubital), large enough size (2-2.5 mm) and presence of large perforating veins (2 mm), then they are better candidates for an endovascular approach. The doctors also discuss the types of devices used to create the anastomosis for the fistulas, such as WavelinQ and Ellipsys. 👨⚕️🚀 Hydrocephalus, derived from the Greek words "hydro" meaning water and "cephalus" meaning head, refers to the abnormal accumulation of cerebrospinal fluid (CSF) within the ventricles of the brain. Despite its name implying "water on the brain," it is the buildup of CSF that characterises this condition. 🧠 The underlying causes of hydrocephalus can vary. It may result from increased CSF production, decreased absorption rates, or obstruction in the normal flow of CSF between the brain and spinal cord due to pathology. CSF, a clear organic fluid circulating within the cranial vault, serves crucial functions. Acting as a shock absorber, it protects the brain and spine. Additionally, CSF provides nutrients, facilitates waste disposal, and regulates optimal intracranial pressure. Hydrocephalus presents challenges, and there is currently no known cure. It is more commonly observed in infants and individuals above the age of 60. While no cure exists, medical and surgical interventions offer effective management options. Patient had multiple episode of seizures in last 6 months. CT head, mri brain, DSA observations of malformation matched with intraoperative findings. patient made complete recovery. Check CT angio showed complete excision with filling of all normal vessels. Spider leg appearance and trapped temporal horn sign in #Tuberculous #Meningitis (TBM) MRI T1 contrast axial sequence showing spider leg appearance ( marked red arrow ) and trapped temporal horn sign ( marked magenda arrow) . Thick gelatinous exudates around the basal cisterns and sylvian fissures appear as intensely enhancing areas with a ‘spider-leg’ appearance. On neuroimaging, the ‘trapped temporal horn’ sign is suggestive of focal obstructive hydrocephalus at the foramen of Munro. The British Medical Research Council classifies tuberculous meningitis into three stages based on its severity . Stage 1 (mild) describes patients with no focal neurological signs. Stage 2 patients exhibit moderate neurological deficits such as cranial nerve palsies or hemiparesis.
