Urogenital emergency 🚨: Paraphimosis 📌 Def: Protrusion of penis outside the preputial cavity and inability to turn it back; penis may appear swollen or in abnormal color, hematuria, stranguria, or anuria may be indicated in such cases. 📌 Causes: 1-Early castrated < 3 months dog which resulted in hypoplastic prepuce. 2-Os penis fracture or malformations. 3-Chronic priapism or prolonged sexual excitement. 3-TVT 4-Congenital shortening or stenosis of prepuce. 5-Retractor penis muscle paralysis. 📌 Prognosis: good to guarded according to severity, may lead to severe necrosis and penile gangrene. 💊 Treatment: 1-Rinse prepuce with saline with a hyperosmotic agent (dextrose 50%) to decrease its size, cold pack is also used for the same purpose. . 2-Heavily lubricate the exposed portion of the penis and try a manual replacement with purse-string suture for 1-24 hours (In some cases the preputial incision is required). . 3-If the penis is severely traumatized and urethral involvement is suspected, place an indwelling soft rubber urethral catheter for 7–14 days. . 4- Sedation or general anesthesia to clip all hair around the penis and debride necrotic tissue, penile resection, castration, and urethrotomy are necessary. . 5-Phalopexy Credits: -Clinical Veterinary Advisor Dogs and Cats 4th ed. -Emergency Procedures for the small animal veterinarian 3rd ed. -https://lnkd.in/di76g2sg -https://lnkd.in/d363MvT2.

Use of asprin/acetylsalicylic acid post calving Postpartum protocols to help cows in getting pregnant early always fascinate me a lot. Recently, I was listening to Dr. Barragam who was talking about transition inflammation management and use of asprin. He mentioned that using asprin at dose rate of 100mg/kg after every 12 hr interval of calving for four times help cows in tackling the postpartum inflammation. Further, cows in asprin group had fewer mastitis cases, constant milk production and got pregnant earlier. Further, Dr. Barragam said that it is not a practical approach to give asprin to every cow which is calving. So he mentioned three conditions in which giving asprin is going to benefit the cows; 1. First calvers 2. Cows having high bcs (>3.75) 3. Cows having dystocia However, in Pakistan Asprin is only available on human medical stores and these preparations contain only 300mg of asprin in it. It will be impractical to give 200 tablets of 300mg asprin to a 600kg cow after every 12 hr interval. So, I would like to suggest the researchers and veterinary medicine manufacturers here in Pakistan to work on this aspect and further come up with more practical options.

Common eye problems in dogs and their treatments: Conjunctivitis (Pink Eye): Conjunctivitis is the inflammation of the conjunctiva, the clear membrane covering the eye's surface and inner eyelids. It can be caused by allergies, infections, or irritants. Treatment: Antibiotic or anti-inflammatory eye drops, ointments, or oral medications to treat the underlying cause and alleviate symptoms. ie Tobradex eyedrops Corneal Ulcers: Corneal ulcers are open sores on the cornea, which can be caused by scratches, foreign objects, or infections. Treatment: Treatment involves topical antibiotics, pain relief, and sometimes, an Elizabethan collar (cone) to prevent further irritation and allow the ulcer to heal. Cataracts: Cataracts are the clouding of the eye's lens, leading to vision impairment. They can be caused by age, genetics, or secondary to certain medical conditions. Treatment: Surgical removal of cataracts is often the only effective treatment to restore vision. However, not all dogs are suitable candidates for surgery. Glaucoma Glaucoma is a condition characterized by increased pressure within the eye, which can damage the optic nerve and lead to blindness. Treatment: Treatment aims to reduce intraocular pressure using eye drops or medications. In severe cases, surgical intervention may be necessary. Cherry Eye Cherry eye occurs when the gland in the third eyelid prolapses, causing a red or pink mass to protrude from the corner of the eye. Treatment: Surgery is usually required to reposition and secure the gland back into place, preventing further irritation and potential complications. It's essential to seek professional veterinary advice and care if you suspect your dog has any eye problems to ensure appropriate diagnosis and treatment. The use of the Erector Spinae Plane (ESP) block in dogs undergoing hemilaminectomies is emerging as a significant advancement in veterinary medicine. This technique, primarily aimed at managing pain, shows promising results in reducing the need for opioids while maintaining effective analgesia. A study focusing on dogs undergoing hemilaminectomy surgery compared the ESP block with traditional opioid-based treatments. The results were quite revealing. Dogs that received the ESP block showed a notable decrease in perioperative opioid consumption. This is particularly significant because it not only helps in managing pain effectively but also reduces the potential side effects and complications associated with opioid use. Moreover, the ESP block was found to lower the use of intraoperative adjuvant analgesics like lidocaine and ketamine, suggesting a more comprehensive pain management approach. These findings are critical, considering the opioid crisis and the ongoing efforts to find effective, non-opioid pain management strategies. In veterinary medicine, where communication about pain can be challenging, the ESP block appears as a valuable tool. "The veterinary medicine xylazine is an ever-increasing additive in the US’ illicit fentanyl supply. In addition to the substance’s adverse effects on the central nervous system, xylazine use often causes severe cutaneous and necrotic wounds due to its vasoconstrictive properties...Wounds range from partial- to full-thickness injuries and may involve large regions of muscle necrosis, bone exposure, and/or osteomyelitis. Wounds quickly develop eschar, and several wounds in proximity may connect through tunneling. They have a high secondary infection rate and become necrotic very quickly, and may develop abscesses, sometimes within days of presentation."
