Is this new PRP dosing math? Help! I just got this from a patient who had a procedure at another clinic. I asked for the clinic note to see what was done so I could better advise the patient. Here is what the doctor told me: He drew 156cc of blood with 24cc of ACD-A....total 180cc Starting platelet count of 281. He created 15cc of PRP at a platelet count of 4630 (yes that is right). I had to stop there because the Maff does not work. Even if the initial draw was all blood the total platelets would be 180cc x 281 = 50.6 Billion platelets. The PRP numbers he gave me: 15cc x 4630 = 69.5 Billion platelets. So the process created nearly 20 Billion platelets! This is more than a rounding error in my opinion. I want this centrifuge 😀

The basic concept is similar to using a tag to close a bread bag: The skin of the sac is rotated, then locked with a clip. In the case of LAA, the process stops clots from escaping into the bloodstream.

Ophthopedia Update:Association between metformin use with circumpapillary retinal nerve fibre layer thickness and capillary vessel density in glaucoma: Background/aims To investigate the association between use of metformin and circumpapillary retinal nerve fibre layer (cpRNFL) thickness, as well as whole image capillary density (wiCD), in patients with glaucoma.Methods This cross-sectional study included patients with glaucoma suspect or primary open-angle glaucoma (POAG) underwent optical coherence tomography angiography imaging. Use and duration of antidiabetic medications were assessed at the time of imaging. From the #clinicaltrial trenches: Dumb stuff is costly too. Imagine a mandatory PK visit. The lab kit includes five tubes. Three of the tubes have colored tops appropriate for routine blood work - consistent with every other routine blood draw and the lab requisition. There are two additional tubes. Both tubes have the same top color. Their top color is different from the others. The lab requisition requires labeling of two tubes for the PK draw. 40% incorrect. The PK kit, with two different tubes, quietly rests 30 cm from the regular visit kits. The person who usually pulls the visit kits was away. When I was told why the kits were separated, it made sense. A lapse in internal communication (or equally likely – some would say more likely, lapse in my hearing – some would say understanding) resulted in a costly mistake. #clinicaloperations #clinicalresearch #drugdevelopment

Ophthopedia Update:Impact of residual retinal fluid on treatment outcomes in neovascular age-related macular degeneration: Treatment decisions for neovascular age-related macular degeneration (nAMD) in the setting of individualised treatment regimens are adapted to disease activity. The main marker of disease activity and trigger for re-treatment with anti-vascular endothelial growth factor (anti-VEGF) agents is the presence of retinal fluid on optical coherence tomography (OCT). Recently, attention has focused on the impact of residual retinal fluid on nAMD management. Based on a literature review and the combined clinical experience of an international group of retinal specialists, this manuscript provides expert guidance on the treatment of nAMD according to fluid status and proposes an algorithm for determining when to administer anti-VEGF treatment according to residual fluid status. We explore the role of residual fluid in treatment decisions and outcomes in nAMD, taking into consideration fluid evaluation and, in particular, distinguishing between fluid in different anatomic compartments and at different stages during the treatment course. Current limitations to identifying and interpreting fluid on OCT, and the assumption that any residual retinal fluid reflects ongoing VEGF activity, are discussed. #Ophthalmology #Ophthotwitter #BJO

CONCLUSIONS: KCMO measures GDMT intensity by incorporating dosing and treatment eligibility, provides more granularity than existing methods, is easily interpretable (percentage of ideal GDMT), and can be adapted as performance measures evolve. Further study of its association with outcomes and its usefulness for quality assessment and improvement is needed.
