The incidence of ulnar collateral ligament (UCL) injuries in overhead athletes has increased drastically over the past few years. 1. Common reasons may include: pitching over 100 innings per year, pitching while fatigued, pitching with higher velocity, pitching on consecutive days, and pitching with a glenohumeral internal rotation deficit, among many other reasons. 2. If you are noticing persistent medial elbow pain in combination with a consistent decrease in velocity, you should be assessed for UCL insufficiency by a licensed medical professional. 3. Special tests to assist in a diagnosis most commonly include the valgus stress test and moving valgus stress test. 4. Diagnostic imaging (the next step in the process) would provide a more in-depth and accurate visualization of the tissue. 5. There is still debate regarding whether the UCL repair or UCL reconstruction is more effective. However, these options should be discussed with your provider. UCL injuries are NOT the end of the road! There are plenty of success stories of players returning to their prior level of function, but it takes the proper plan and medical team to follow through on this goal! Jang SH.
